Not expert but I believe you crashed it by freezing it. Basically caused hormone to separate from carrier oil. You will have to heat it to recover it. Others more experienced will chime in. You can also search for crash or crashed test and find instructions how to recover.

Only by knowing both. The mg is listed usually as per ml. Example: A 10 ml vial of test cyp 250 is saying 250mg per ml. 250mg/ml x 10ml is 2500 mg in whole vial. So if injecting 125mg u would inject 1/2ml to obtain that dose. Also 1ml= 1cc. Hope helps
