Diet and training definately aren't the issue. I'm very strict with my diet, measure out all the food I eat, never have cheat meals, etc. Training is good as well. I've gained 100 lbs to my squat in the last year, I'm doing my 1rm for deadlifts from a year ago for 10 reps now.

Have you tried DC's calf training protocol? I guarantee it is the most painful calf exercise you will ever do. Its 3-5 mins of constant tension on your calves.

I only train 3 days a week as well. I feel the same way. I could train 4 maybe 5 days a week but after a week or two I feel burnt out. Training 3 days a week always keeps me feeling motivated and recovered.
