In range Total Test, "HIGH" Free Test...wtf?

Hey guys,

I've been on testosterone replacement therapy (TRT) for 3 years now and have been at a nice happy 700-800ng/dL total test with an in-range free test every time I get a blood test as part of my testosterone replacement therapy (TRT) regimen. This has worked beautifully with all my other results coming in at normal levels and even some on the optimal side as I've been working really hard to get as healthy as possible. What I find perplexing is that I received a phone call from my doc's office saying that they need me to come in as my bloods came back in and for some reason my TT was 908 (that's normal considering the pin was done in close proximity to the draw) but my FT was 313pg/mL! (Range of 35-155pg/mL)

I've been doing a lot of reading to see why on earth my FT would suddenly spike like this and the only thing I can think of is that my SHBG has decreased, allowing me to keep more of the test floating around in my bloodstream. I don't see any issues with this elevated value personally, but my doctor is one of those "numbers" guys that flips out when he sees that HI on a blood test. Does anyone know *why* this would happen or if perhaps I'm overlooking something? I have been getting over a doozy of a cold recently so I haven't been to the gym in a little over a week, but I doubt that would cause this...

Additional information:
Total Test: 908ng/dL (250-1100)
Free Test: 313pg/mL (35-155)
Estradiol: 29pg/mL (7.6-42.6)
SHBG: (Plan on having him order this)
CBC/RBC/Hematocrit: in range (on different report that's in the car - d'oh!)
PSA: in range
Others are all in range such as lipids/metabolic panel/etc.
Note: I am a type II diabetic and I know that insulin resistance can play into this, but I thought that it caused SHBG to elevate.

Your thoughts?
 
Dam it I dont know what any of that means but im going to you for help when I get on testosterone replacement therapy (TRT) it may be soon :)

Man, it's funny how much time you spend learning about this stuff when you HAVE to inject. I just feel like I got "busted" by my doc that really knows very little about testosterone replacement therapy (TRT) and he's going to play with my dosages because having a high Free Test is spooooky. I'm hoping to find a reason for this so I can explain it to my medical professional... D'oh!

Sucks that there aren't any really good docs in my area that understand testosterone replacement therapy (TRT) and I'm unable to afford the pros right now at Increase My T. I'll definitely help you to the best of my ability though! :)

What part of the country you in? I thought the same for years until I left Iowa and started looking a few hours away in Nebraska. Found a really great testosterone replacement therapy (TRT) doc who works at the Nebraska Med Center. You'd be surprised where you can find these guys lol.

I live in the Southwest amongst the saguaro cacti and bark scorpions. ;) While we're starting to see more and more docs that are willing to treat hypogonadism, most of them are only doing so because of the high percentage of retirees down here, and aren't really (in my opinion) qualified to do so. The ONLY reason I'm with my doc is because he actually listens to me and is willing to order whatever tests I want based on my research. He isn't willing to handle AI's/HCG so I just "use" him for the 250mg/wk script for testosterone replacement therapy (TRT) and follow up support.

The only available endo in my area is a diabetes specialist that graduated med school in the 70's and feels that testosterone replacement therapy (TRT) isn't really necessary but was willing to carry my script after having me discontinue my testosterone replacement therapy (TRT) for 2 months to get his own baseline tests done. After several years at <200ng/dL of total test, no way am I going to go back to that - even if only for a few months. :thumbsdow
