Question about Free T and its relation to TRT.

Free T is used when evaluating whether or not someone is hypogonadal if their Total Testosterone is considered to be "borderline".

Once someone is on TRT, it doesn't seem to be used much in the monitoring process. I suppose one could say it is too high if it is above the normal range, but I personally haven't noticed any problems when it is above normal. Unless of course you consider high libido a problem. :-)
 
Free T is crucial to TRT. Having a high level of circulating loosely-bound testosterone molecules means there are more molecules available for activation at the receptor level.

Plenty Doc's won't even look at Free T for diagnosis of hypogonadism. Which is totally wrong.
I was border line low T.T. and my Free T would come back below range. Endos would be first to chime in there 2 cents saying it's not important in diagnosing Low T. Totally wrong. If it's low it's low.....
There's a reason for it being low.
 
To partly answer OP question...
I think Free T is bound to a different protein like Albumin. No doubt it will come back at a higher level on blood work if you are testing it being on TRT. I know this from myself.
Bio Available T is a more accurate diagnosis and is more commonly used by docs.
If SHBG is considered "high" or its elevated slightly it could lower circulating T even if T.T. is decent.

It is definitely time that I find a new doctor. My TT is at 560 but because my free T is at 18 he is not willing to increase my dosage because he said my free T is high enough and that they (TRT Clinic I go too) are more concerned about Free T than TT. He also said that my Sex Horm Binding Glob is low so this is the best I could expect. My Sex Horm Binding Glob is on the low side of the scale but still within range (17.5 - 16.5-55.9) so I am very confused.
