I think there's an interesting question here. Clearly, evolution and/or biology has determined that testosterone levels fall as we age.
But is there actually a good reason for this? Logic suggests that evolution stops caring about us...
Maybe look into DHEA and Pregnenolone? My understanding is that HCG is partially used to keep these present on TRT, but that obviously won't work in your situation.
You could get a full hormone panel workup privately; that might furnish some clues.

Your test concentration is probably 200mg/mL, so you're probably using 100mg per week by injecting 0.5 mL. Maybe double check what it says on the vial?

Most likely PVCs - premature ventricular contractions. This is generally considered benign until the percentage of PCVs becomes high. I found them disturbing and uncomfortable though. When I had these I had myself checked out with a 24-hour monitor, ECG and ultrasound.
