Any known debates regarding appropriate use of HRT/TRT?

Does anyone know of any links, videos or published debates regarding the appropriate use and/or misuse of HRT/TRT?

It became pretty clear to me early on when researching Hormone Replacement Therapy (HRT) that there are two schools of thought when it comes to how treatment should be administered and to whom. With LowT clinics and 'aging centers' popping up all over the place, I assume that these dissimilar views have been subject to some form of open discussion.

For example, Dr. A thinks that putting someone in the high-end of a range (which they admit is not historically static and defined by insurance companies) just to try and increase mood or quality of life is "bad medicine" while Dr. B basically says 'whatever ... there is no research to back the negativity and it makes peoples life's better, so pin up!'.

Just curious ... all I have ever been able to find is doctors trashing each other and negative media/stereotypes/misinformation.
 
It's not a "professional" opinion or anything but here's how I see the split.

It's not that normal docs consider it "bad-medicine" to prescribe higher doses of test, they just have no incentive to do so. They work for the insurance company, not you the patient. They do what the insurance companies tell them to do which in most cases is to cost the insurance companies as little as possible. A GP for example can request everything in the world, bloods, scripts, scans whatever but the insurance company can turn right around and just say "NO". At which point the doc's hands are tied.

It's also a liability thing. I would argue that most GP's and endo's are merely trying to give you the least amount of treatment possible to get you back in the "normal" range so that they can get you out of their office. If they were to up the dosage and start experimenting with quality of life, if something were to go awry their ass would be on the line. It's sad to say but protecting themselves from malpractice suits seem to be their priority.

There is also the fact that the literature that these insurance docs are using to treat Hormone Replacement Therapy (HRT) patients is extremely outdated.

The anti-aging and wellness clinics get to play by different rules because they don't go through insurance.
Therefor they work for you the patient and will do what's necessary to keep you on their books for as long as they can.
