I've been on TRT for one year. I started on gels then went to injectables 8 months ago. I have been on 200 mg test cyp split in 2 doses per week, 500 iu HCG 2 times a week. I've been on and off anastrozole trying to get it right.

Around 6 weeks ago my blood work showed my TT at 1467 and e2 non sensitive at 14.7. This was drawn 24 hours after injecting. I got antsy about the high TT so I lowered my dose to 160 mg a week and stopped the anastrozole. Around 3 weeks ago I started developing Acne on my backside and it isn't going away.

So how do I manage this? Any advice would be appreciated.

thats your issue, starting and stopping an AI. why would you? if estro is high you up AI a bit, if its low you drop your ai dose down a bit, till you get it right and even . you dont just stop it. what do you expect to happen if you already needed it to begin with. now you are just causing hormone fluctuations....

Agreed. When I flux things quickly - I break out. I also break out when I push T too high - despite managed estrogen levels, my skin just gets really oily.

Just recently, I was uh... a little saucy on a Saturday night, and did a back to back nights of T shots on accident. The bump in T was enough to cause a breakout on my back that took a week or two to resolve.

I've used this combo with what I think is success when I have a breakout on my back - Pantethine (B5) 750 mg with 250 mg of L Carnitine each three times per day.

14.7 on a E2 non sensitive is very low.
I had BW done E2 non sensitive came at 22.3 and on the sensitive test was 9.6.

Have you run blood work again yet?

Not yet and I've kind of screwed myself in being able to fine tune a protocol because I have been changing too many variables based on how I feel rather than staying the course and waiting to see the results of bloodwork

That is exactly why you've broken out - rapid fluctuations in both test & estradiol are a well known cause of acne. There are plenty of basic OTC treatments available for minor acne but until you get your protocol dialled in - these options aren't going to be fully effective.

Stick to a protocol, wait for the bloodwork, make SMALL adjustments as necessary - rinse and repeat.

Could high hemoglobin cause acne also? I had broken out real bad on my back, after I donated blood and got back in range they started disappearing.

Not that I am aware of. And donating blood would have lowered your hemoglobin instead of raising it.

The only thing that really works if your prone to acne is accutane everything else is a temporary solution I've taken it before and was clear for a year my acne returned but it's almost non existent compared to what it used to be,however I'd never recommend it to anyone
