Help With Doctors Decision

Hello fellas! 35 year old male here and was looking for opinions on what you all think on some labwork ive been getting recently. So since 2013 Ive been getting checked yearly just for in the know of what is going on in my system. Everything is great except that Testosterone serum has always been riding between 360-400, which is "normal". This year I got flagged on free testosterone direct which was at 5.7 in a 8.7-25.1 reference range. The Test Serum was at 282. I had noticed my libido drop and erections were not that great for the past month. Work stress related maybe? I was sick with a stomach bug shitting the night away the night before the exam which im sure didnt help my case. My doctor referred me to a Endocrinologist. He made me repeat the test serum and free Test again.

6/12/2020 Endocrinologist
Test Serum - 367
free Test - 6.4.
SHBG Serum- 29.4.
LH - 7.5
FSH- 2.6

He mentioned he wanted to have a sleep study done to rule out sleep apnea. Libido is there but its not what it used to be, which was a damn wanna be pornstar with my wife only haha. He stated that im still in the good range and wants to find the root of the problem and not band aid fix it with jumping on TRT. Would these levels be causing libido issues and feeling quite irritable for the smallest things? What do you all think about my situation? Sorry for the bad grammar/layout. English is my second language.

I do work out 4-5 times a week. Weight is at 175 at 5'7 with about 17% BF. Eating habist is ok not the best.

I mean that was over a year ago. I already had 4 labs done since then starting Feb of this year. I was back to my “norm“ test serum range of 400. Not sure what you are asking though, I used Clomid/Nolva combo for 4 weeks but like I said over a year ago (2018)

folks get back to normal levels using gear is what i am saying or they stop it alltogether and recover naturally as nature intended.

Your values are still considered as "normal" as your doctor said!
I'm in your same situation since my testosterone never exceeded 4.9 mg/ml and it usually ranges from 2.98 to 3.5 mg/ml, sometimes the free testosterone is very low, but all the doctors told me that my values are still good.

Obviously, this is not true, good testosterone values for our age should be at least 6 mg/ml.

Have your doctor suggested you to pin 500mg/week and was it just your idea?

This is one of those areas where you get told you are normal, but are at the bottom of the range. 400 is a level where some people start to have symptoms of low T. Everybody is different, some could tolerate lower and be fine, others would be a wreck at that level.

I would think low libido, impacted performance, irritibility, and loss of aggressiveness would be markers.

Your doc is doing the right thing trying to find the root cause. But just be aware that even if he fixes you up, you'll not likely go above the 360 to 400 you mentioned. So you'll have to decide if that's good enough or not.

I'm no doc, but when I see your LH at 7.5 and test low I'd suspect your nads are underperforming. One pretty easy to do and low impact thing to try could be doing clomid mono therapy. Maybe see where your doc is going with this, and at the right time discuss that option versus others.
