Testosterone replacement therapy (TRT) and fertility?

TRT and fertility?

Hi folk,

I'm in the UK and have been on testosterone replacement therapy (TRT) for a few years - Testogel 5mg one per day, with no Human Chorionic Gonadotropin (HCG) or Aromatase inhibitor (AI), as they just don't prescribe them here in the UK! The testosterone replacement therapy (TRT) knowledge here is very poor and I was never properly diagnosed with primary or secondary... secondary, I suspect.

We would like to try for a child soon and so I had my sperm count checked just to see. Unfortunately it wasn't good, 1.2million (anything under 20 million is bad...)

I'm heartbroken by this, I was told by my urologist that fertility should not be affected by TRT!!! He clearly doesn't know what he's doing...

I'm looking for some help/advice on how I can try to improve my sperm count without the help of the useless doctors in the UK.



Is it possible to stay on a low dose testosterone replacement therapy (TRT) and add a low dose Human Chorionic Gonadotropin (HCG) to improve fertility?

Would HMG be a must? It would be very hard for me to get, if at all...

Is coming off testosterone replacement therapy (TRT) my only option?

What about a Strong PCT with high dose HCG, Clomid, Nolva etc?

Any help, appreciated, thanks!
 
I had the same issues, I had to quit the test, after 2 weeks I took clomid for a few weeks and the sperm count went way up. I only took clomid for about 2 weeks due to the crazy side effects it was causing, but if thats what it takes to get it done its worth it.

Fertility is OFCOURSE affected by HRT/TRT!

you have too low of levels so you go on hrt (this now shuts down the levels you did hav commign out to varying degrees)

it is still possible to have kids on hrt (same goes for on cycle) with or without HCG, but you may have a better shot with it.

tenn2525 - two weeks seems a very short time to increase your sperm count on clomid? Did you have sperm counts done before/afterwards?

Any other advice or experience appreciated!

There are at-home sperm-check tests the have been building credability. I've seen simple ones for $30, and ones with microscopes for $80...all online. Try different pct methods and buy a test you can re-use multiple times and check frequently for progress...jmo. Regardless what you do/try, it doesn't sound like it's going to be an overnight solution
 
well your not completely sterile and there still is a chance. But your best bet is to just come off of T for when your trying to conceive.
 
Well, I saw my urologist yesterday. He wants me too go cold turkey, off the testosterone replacement therapy (TRT) for three months with blood tests every month before he will put me on Clomid for fertility!

Personally I don't want to be on nothing for 3 months, I don't see the benefit of that, I'd rather be on the clomid now but he was having none of it. I can't face feeling as bad as I did before testosterone replacement therapy (TRT) again.

I'm considering putting myself on clomid now, with some HCG. Can anyone comment on this or offer up help with doseage for this? thanks.
 
I would be interested in reading a testosterone replacement therapy (TRT) post cycle therapy (pct) for sperm count as well. Also, is there a hcg, or other, regiment someone could take while on testosterone replacement therapy (TRT) to help make sure sperm count isn't lost permanently

Well hes probably having you go cold turkey because he doesnt have numbers before you went on testosterone replacement therapy (TRT), and figures if you are fine naturally theres no sense on going with clomid. From what i have seen doing clomid and HCG at the same time isnt the norm and my doctor doesnt approve of it. But if you were to go on HCG it seems most people do 250iu EOD or 500IUS twice weekly.
 
Anything synthetic shuts your natural production down, that being said, HCG acts as a substitute to LH, there for keeping your testicles full. Clomid is a stimulant to get your body to start producing its own LH again, therefore naturally starting up production again. You dont run the two together, this is counterproductive.
HCG is normaly run at aprox 500 iu's a week, or blasted at the end of a cycle, not sure on the dosages here, aprox 1000 iu's EOD?

Clomid is started after everything is out of your system, depending on the half life of the compound you were on, usualy two weeks. This is usualy run for 4 weeks at varying dosages about 100mg's to start.

If its been two years, its a good chance your pretty well atrophied, so your going to have to give them time to start back up, but everyone is different
 
Thanks guys. I will just go cold turkey, as he asks, start on the clomid after 3 months and pray... If things get really bad, I can always get an appointment sooner and ask again for the Clomid then.

If I go cold turkey for the three months and have some blood tests to show my T has dropped a lot, I get clomid on prescription from my doc, which I would rather do for various reasons. But I'm not sure I want to go through those three months of feeling as bad as i did before TRT!

Or, I self prescribe now and get the ball rolling.

I don't know what to do....

If I could self prescribe clomid now and still somehow manipulate my Test levels around blood test days over the next three months to show T dropping - then get the clomid on script, but is that possible?

What dose did your Uro recommend 300winmag?

I would stay on HRT and add clomid 35-50mg ed for 3-6 weeks and in that time try to have it take.

your right about libido, and why i rec this first.
hope on clomid for 3-4 weeks and try to give it a go... and also for another test to see if it helped if you feel it is not working.
I would no take clomid nonstop for months on end.
but 1-3 to try and have a baby is not a bad idea. I have read studies where this was used in research with mostly positive findings in males subjects. these where subjects on HRT BTW.

HCG 500iu 2X a week (about every 3-4 days) for 4-7 weeks
clomid 35-50mg ed for the same time, after 2-3 weeks try giving it a go and again at 6-7.

then possible continue Clomid 2 weeks longer with out hcg and give it a 3rd go.

I feel bad for you that you are stuck with this doc.
he will pull you off hrt but not give you clomid right away? it dont make sence to me man...

he is not ON CYCLE he is on Hormone Replacement Therapy (HRT), this is not the same case, you are wrong.
yes it shuts you down, but he need test and is not "on cycle" so the first option is NOT to just jump of Hormone Replacement Therapy (HRT), thats the last option.


DONT come off HRT yet. just add the hcg and clomid.
i have for my self seen more then one study on many people on HRT trying to conceive with and without clomid and i can tell you there was a VERY good positive effect for the research groups of men on hrt that where given clomid+ hrt over the group just on hrt.

this guy is not on cycle...

so you are on a cream not injections?
if so stop 1-2 weeks before blood work and they should drop. but keep ion mind at the same time clomid may be raising them. but so what tell him you forgot a few days before out of routine and put some on before you remembered not to, but that you didnt think it would effect the test.

get some clomid now and run for 4-6 weeks is what i rec, and if not a go, try with hcg also. then go from there. but i have a feeling JUST the clomid and HRT will help you out with what you need.
