flat i hit a little under my nipple, incline i aim for just past my chin, decline i hit about 3 or 4 inches under my nipple... im measuring with my fingers so bare with me on that measurement
I reverse grip about 90% of my flat bench after doing it on tricep days for awhile. I go to about 2" below the nipple regardless of method, if I go higher my shoulders will bitch. I posted an article which covers this in this thread:

i switched when i blew a shoulder out benching conventional . it still hurts to go heavy overhand but its smooth sailing reverse grip .im probably 4-6 inches below the nipple when i touch , ive never measured lol . i know i dont get as much chest work from RG but my delts and tris get hammered and grow/get strong .
