Back Injury

6 Months In...
Hi All,

Don't know if there is a better place for this thread? Please move if there is.

I'm still suffering from an injury I got deadlifting 8 months ago. I felt a sudden pain in my right lower back. I stopped an went home.

8 months on and I still have the same symptoms which are:
Stiffness and pain in my lower back when leaning forward or after sitting or walking for a period of time
Pain around my hip and down my leg to my lower quad (right side)
Pain sometimes along my groin (right side)
Lower Back spasms
Very tense muscles in my lower back
The main source of the pain is just above my right glute.

I know this sounds like Sciatica (but these are the symptoms, I want to know the cause). What is causing the pressure of the sciatic nerve or is it something else causing the pain and problems? Discs, muscles, ligaments?

I've seen a Chiropractor who has helped relieve some pain and tenseness, but hasn't given me a diagnosis. Nor has a doctor. I had X-rays taken yesterday and get the results next week - I don't believe they will show much and will hold out for an MRI.

If anyone has had anything similar or any suggestions as to what it might be or what I can do, I'd love to hear them.

Thanks.

sounds like you're going to need a long recovery period.

could be anything at this point, probably a disc bulging or worse.

have you been iceing it every night? if not, try it.

have you put heat on it? i mean, there are only so many things you can do, try them and see if it helps at all.

what is have your workouts been like, if any?

MRI for the win, xrays wont show shit

has the chiro got you on a rehab program or are they just manipping you?

If I was to guess I would be saying L4/L5 posterolateral herniation protruding on L5 nerve root. Are you getting any weakness , numbness or mm wasting?
But without doing a proper exam its just a guess

we can sit here and guess all day, but MRI should give you a definitive answer.

I stopped working out for around 3 months. There was a slight improvement from manipulation and rehab. Then I started working out again (probably a big mistake :-/ ) I stretch daily and use strengthening techniques.

I've not deadlifted since, but my squats have been fine. Strangly enough, overhead press and incline bench seem to aggravate more than anything (probably cos they compress my lower back). I don't seem to suffer any more or less after working out. I just thought I better stop to try and get it healed.

I know I need an MRI - but trying to get one on the NHS (i'm in the uk) isn't as easy as it sounds - I may go private.

No numbness. No muscle wasting - just spasms and pain. I've been using hotwater bottles & heat massagers.

Not sure if it gives any clues, but the injury occured pulling the weight up, just before straightening my legs. My guess is I used my back too much, rather than my legs for the first part of the movement.

I'm hoping for a muscle tear or ligament strain rather than a disc problem! What is the treatment for disc hernation? Surgery.

Thanks for the replies guys, anyone else got any thoughts?

well sounds like inked was on the mark.maybe just herniation of the disc.inwhich if not ruptured then some good steroidiol antiinflamatory meds and rest rehab should do it.if a rupture a would say maybe even still some good anti inflams and rest rehab could do the trick, but a discectomy maybe needed.not a huge deal pretty good recovery from that type of surgery.

back paimn

I have a problem with my back that sounds alot like yours.
If it's been hurting for 8 months, It's probably not going away on it's own.
After Xrays and MRI, thay found L4 L5 and L5 S1, losing thickness with the L4 L5 having a bulge.
The pain is probably from the bulge but both disk are not good.
I've had 3 shots and a couple months of PT, with little relief.
I stopped lifting about 9 months ago while the shot and PT were being tried.
The surgin I've seen about it said to fix it for the long term, a double disk replacement is the answer.
Scarey!!!!
It's done all the time but is still a major deal.
The problems started small, a couple of years ago.
I'm interested in starting back at the gym being carefull with the back.
I think that a strong body will help in recovery when the time comes for the big fix, which I'm putting off for now.
Not trying to scare you but when I first went to get checked out, I thought a little stretching would be the answer.
Then they hit me with the real problem and fix.
That knocked me for a loop.

surgery should always be a last resort

its a good sign that there is no sensation or strength deficits ,honestly mate it is worth forking out some money to try and resolve this problem. You only get one body, why not look after it

Thanks Inked. I'm not scared of kicking up a stink or sticking some cash up to get it sorted - at the moment I'm just after the correct diagnosis. I've just been looking for the opinions of guys that are likely to have done similar things, opposed to a doctor who has never lifted in his life. I should know more from a specialist next week.

You reckon avoiding the gym for a while is a good move then?

Carsons - I understand your point, but if surgery is the answer then so be it. I've already had surgery for Perthes diseas (mis-shaped hip bone on left side) and an anterior cruciate ligament reconsrtuction - both of which clearly had something to do with my recent back troubles. My Perthes op was way bigger than this - if i'd not had that I'd be in a wheelchair by now.

wow thats pretty big,how old were you when you had the hip surgery. was it only on one side?

I would keep training, but stay within your pain limits ( ie you should have no back pain when training).

is the pain worse when standing and bending forward, or leaning back?

I was about 12 when I had the surgery so didnt really understand what was going in. The ACL recon was 3 years ago. It was just my left hip and my right knee.

I have no back pain directly from training or during. Sometimes i have increased stiffness and pain the morning after OHP, incline bench or squats. I cant lean backwards cos my back will just go into spasm past a certain point. When im sat or walking i tend to get the pain just above my right hip bone and sometimes into my groin. There is no excess pain when bending forward from my waist/pelvis, but if bend from slightly above my waist I get pain across my lower back.

Perthes is an interesting disease (not for the patient though). have you had any hip problems since? Did they replace the head of femur?

the back symptoms sound pretty typial of an inflammitory condition. From what I can tell , you could try lying face down on a falt surface(bed or the floor whatever). have your arms in the push up position, now move your hands about an inche or 2 closer to your body.

Keeping your navel in contact with the ground, push up through your arms, arching your lower back. Your arms can remain straight.

you should feel this through the mm in your lower back , and may initially be painful.

hold at the top for 2 secs and slowly lower. repeat this ten times.

this repeated extension movt should hopefully help with the symptoms. If after doing 10 or so it aggrivates them, stop. It is normal for the first few to be painful, then the pain to slowly get better

I'm not 100% sure what I had done to the femur - I believe I had something pinned to it to create the ball shape - as you may know Perthes is a flattening or smoothing of the ball at the top of the femur. I still have the pin in a jar at home! I have had almost no problems since the treatment other than a little cramp.

You say infammitory - of what? I assume the inflammation is causing something to press against a nerve (sciatic or whatever) if your diagnosis is correct? Is inflammation likely to stick around for this kind of time? 90% of the pain today is just above my right hip bone, round towards my back (love handle area probably best describes it). Looking around and talking to other people it seems to be a toss up between a hernia/slipped disc or some kinda of muscle tear, with most people suggesting disc problems mainly due to the length of time I've suffered.

Thanks for your help so far bro.

inflammation- of the disc, nerve root whatever. generally the symptoms will improve some what with movt, then become more stiff and painful with rest ( like over night).

I doubt its a mm tear.

go and see a good physio, one that is familiar with mackenzie techniques, and I reckon you will be on your road to recovery. Don't be afraid to call around and ask the physios how they treat, after all it is your hard earned $$, you want to make sure you get someone good

only read the first post....sounds like a herniated disk. How much were you lifting and were you wearing a lifting belt?

No belt - was only warming up. Think it was around 100kg. At the time I was lifting around 140kg for reps. So not even a huge amount.

I've since squatted using a belt and this doesnt seem to agravate the injury.

ink put in some good points.i had the same type injury and had a discectomy to my l4-5.i first used a steroidiol anti inflam and some good rest stretching/pt.it helped a bunch but i was too far gone for it to totaly help.i would try a round of the meds with some stretching/pt.then by all means continue to lift.be smart and don't do things to hurt your back.my docs told to to stay in the gym just do things the right way a strong back indeed does help things.just like you said in a post earlier.

I've been waiting for an appointment beast. Looks like the next available is 30th April. I'm in the UK, so I have to deal with the National Health Service. It's shocking.

I'm very tempted to go private, but I'm not particularly loaded :(
