Blood test results and HCG monotherapy

Little of a back story on me.
I'm 20 years old and have had many symptoms of low T for the past few years now. Brain fog, low energy, no motivation, constantly tired, depression, anxiety, and gynecomastia. I recently went to my GP and told him everything I was experiencing. He ran some blood tests.

5/03/13

Testosterone Value: 259 Range: 241-827 NG/DL

Prolactin Value: 9 Range: 2-18 ng/mL

TSH Value: 2.03 Range: 0.30-3.80 uIM/ML


5/22/13

Testosterone Value: 232 (L) Range: 250-1100 ng/dL

Testosterone, Free Value: 57.5 Range: 35.0-155.0 pg/mL

LH Value: 5.8 Range: 2-12 mIU/mL

FSH Value: 3 Range: 1-12 mIU/mL

With those results he wanted to start me on 100mg test cyp once monthly. After the research I had already done I realized he had no clue what he was doing. So I moved on to an endo. Endo ran some blood tests.

He said he wanted to see me back in six months to see how my levels change. I don't want to wait around to get my levels where they should be. So on to the next doctor I went. I went to a urologist and had everything checked out down there and he said my testicles feel healthy and normal sized and nothing appears to be wrong.
He said my problem is something that an endo needs to be dealing with not an Uro.

So I said what the hell and gave IMT a call and spoke with Todd. He answered all my questions and was very helpful.... at first. He told me to submit my paperwork and he would get back with me in 24 hours letting me know whether or not I was approved. 2 days after sending in paperwork still no response. So I called numerous times leaving voicemails and also sent a few emails and got ignored? It's been a few weeks now and still no response! Thanks IMT!

Anyways, after that failed I decided to look around my area once again in hopes of finding a good endo. I found a decent rated Endo and set up an appt to see him. He ran some blood testing also. I was on a 10 hour fast.

With those results the endo wants to start me on 1000iu Human Chorionic Gonadotropin (HCG) EOD and come in two months from now for follow up testing. He did not prescribe an Aromatase inhibitor (AI) which I asked about. He said if I have any symptoms of high E2 just to call and we'll go from there. Does this look like an ok protocol for my situation? Does any of the tests other than the obvious ones stand out to any of you that something else may be wrong? Do you think I'm primary or secondary? Will this dose of hCG cause desensitization to it? Having kids is very important to me and I don't want to start something that may jeopardize the chances of that.
Any replies will be much appreciated. Thanks in advance!
 
way to young for Hormone Replacement Therapy (HRT) with testosterone, it will fuck up your chances for kids. not maybe.

probably should try diet and exercise first.

if you must only take Human Chorionic Gonadotropin (HCG) & clomid. wont shut you down. try what the endo suggests. this is a wise first course that he suggested especially if you want to have children

It appears that you probably have secondary hypogonadism and he is trying to get your HPTA working again. I was surprised to see how much your LH and FSH moved over the different lab tests you have had though. You may have bit of primary going on too. Any trauma to your testicles?

Given how young you are, any doc is going to be reluctant to start you on testosterone without trying to get you restarted first. You should want that too. The longer you take exogenous test the harder it will be to restart. You may have to try clomid in your restart too.

If getting your HPTA working again doesn't work, then:-) taking testosterone is probably in your future. If you need it, it is a life saver. It will make you feel alive again. But at your age, it is definitely worth trying to get fired up again. Also, you may want to check your sperm. If they are viable it wouldn't be a bad idea to deposit some at the sperm bank in case you want to have kids later.

Just picked the hCG up and it says for IM use only but my endo said inject into fat? So does either way work? It's an APP product if that helps any.

Also after finding out my insurance wont pay for a single penny of it not even thru a specialty pharmacy its going to run me $220 every 20 days. Its going to be hard to come up with that kind of money every 20 days. That's just outrageous. Definitely going to be shopping around for the next purchase.
 
You should ask your physician why he suggests that amount of Human Chorionic Gonadotropin (HCG) every other day, that is not what we have found works best. There is no reason to dose Human Chorionic Gonadotropin (HCG) more than every 3 or 4 days. Also when using an aggressive dose of Human Chorionic Gonadotropin (HCG) like he is suggesting, you may want to ask him if he has considered what is going to happen to your E2 levels.

HCG directly stimulates E2 conversion and can actually convert directly.

Also know that a restart is different than Mono therapy. Mono therapy only requires 300-500 IU 2x per week to give you optimal TT levels, unless your primary.

My doc told me to take Human Chorionic Gonadotropin (HCG) at 350 i.u. 3 times a week with 0.25 mg ADEX EOD as a monotherapy when I was 25..
8 weeks of that protocol took me to TT of 925 ng/dl
But it doesn't last forever...
4 years later this same monotherapy of Human Chorionic Gonadotropin (HCG) protocol would keep my levels of TT at 519 ng/dl...
LH, FSH, were suppressed while on this.
Although he advised against it...I took time off in between vials refills....

I figured I would give you guys an update on how the hCG is working so far.

Sunday 2PM 1000iu subq in stomach

Only thing I really noticed between my Sunday shot and Tuesday shot was a slight testicle pain and morning wood (didn't happen very often prior to this). The pain would only last for a few minutes but happened a few times. Is this normal? Other than that nothing. No energy increase, no libido, still sleeping 10 hours and feeling exhausted throughout the day. Testes haven't changed in size. Mood and anxiety don't really factor in. I've been on Celexa for the past 3 months but hope to taper off that when I get everything figured out with my hormones.

Tuesday 2PM 1000iu subq in stomach

Still experiencing slight pain in the testes every so often. Morning wood still going strong. Still no increase in energy or libido. Still have the exhausted feeling throughout the day. No increase in testicle size. Last night I was experiencing sore and tender nipples. (about 28 hours after last shot) lasted for about 5 minutes 3 different times. Should I be calling my endo and have my E2 levels checked right away? I already have pubertal gynocomastia and DO NOT want it to get any worse.

Today will be my third shot but I'm kind of scared to continue after having one symptom of high E. Let me know what you guys think!

I can't say how long it takes for Human Chorionic Gonadotropin (HCG) to take effect. But I can tell you that when on Test and Human Chorionic Gonadotropin (HCG) you must monitor your E2. Frequent labs tests are best until you get things dialed in.

You also mentioned sleeping 10 hours and still feeling tired. I can't remember -- have you had a sleep study done? If you have sleep apnea you are going to feel like crap AND it can lower your testosterone. Getting good sleep is crucial.

I just figured my fatigue and exhaustion after 8-12 hours of sleep were from having the same test levels of a 100 year old man.

I was looking over my labs again and the HGH test was standing out to me.

HGH--- <0.05 ng/mL (0.00-8.00)

Is this a low level for my age? I don't know much about the testing ranges for HGH and where I should be but it just seems like a really low number. Any HGH experts out there? Help me out!
 
Get a sleep study done ASAP!!!
I was on testosterone replacement therapy (TRT) and felt like crap cause my Sleep Apnea was so bad!!!

It is all circular and interconnected. Get the sleep study asap.

I am NOT an expert on HGH. But I think I recall hearing that is really fluctuates depending on the time of the day. I think it may get released in "pulses" and remain in the blood stream for something like an hour at best. Hopefully one of the experts can help out.
