I personally wouldn't freak out too much about the creatinine without seeing an albumin panel just yet. Definitely on the dehydrated side of things just looking at the rest of the numbers, but it may just be simply that. If albumin/microalbumin comes back high, then I'd think about talking to a nephrologist (kidney doctor) to see if there's a problem.
The one thing that jumped out at me is your TSH. At 3.5, you are clinically hypothroidic in many endocrinology circles. Are you feeling a little sluggish or having some issues with body fat lately?
Tren is known to hurt thyroid function, but that far out after ceasing the AAS has me a little skeptical whether or not you have had this issue for awhile or not. I do agree on getting that prolactin under control - it can do some nasty things to the rest of your body (like shutting down natural test) if it climbs too high.
