Not sure if you already got the answers you were looking for. If not here are the steps. I just did this and started about a month ago.

Talk to entourage doc. Go to their website and sign up with your info. They send you the info to get your blood work done at a discounted rate.

You fill out the medical forms and doc sets you up. Took about a week and I had my meds.
