I was going to give you guys a try but man lot of back and forth with sending documents over emails and downloads kind of a pain in the ass when it comes to a guy who isn't great on computers. Thats and I was told that I would have to get a physical before being treated which I don't want to do and I'm already on test cyp and hcg so don't see why I would need a physical?

each trt company would need a physcial.. its standard procedure..

the paperwork is not that difficult.. its as simple as faxing it back to them..

and they are the best at what they do.. their prices are very fair too..

beats dealing with an idiot local endo that doesn't have a clue what he is doing 10/10 times..

One of the ways IMT conducts and capitalizes on business is by saving the patient and the physician time and money We do this by managing patient data digitally.

One thing we usually suggest is not to try an email us the forms, uploading them to the website or faxing them is best.

The forms are very similar to the forms you would receive going to see any doctor for the first time. One of the things a doctor must do before he/she prescribes you medication is develop and establish your patient history, so the forms are a must.

You could always print them out, fill them out and then go to your local fedex store and fax them over. This is usually the easiest way for those that are less digital friendly.
