TRT gyno and where to go from here?

I have been on testosterone replacement therapy (TRT) for 9 weeks. I am taking 100mg/wk of Cyp. My initial blood test was for total test and PSA. My TT was 245ng/dl out of 241-827ng/dl range. My PSA was 0.4 out of < or = to 4.0 range. Last week I started noticing slight discomfort and a lump under my left nipple. I've had this before and got it to subside with Letro. I was not running a cycle when I got it before. So I go in for my injection today. I tell the nurse about it. She leaves and comes back in 5 minutes later. The nurse tells me the Dr is not in but she spoke with the PA. The PA said she did some research and the lump was not a side effect of the TRT. At my initial appointment I asked the DR about HCG or AI's along with the Test. He claimed he has never had a need to prescribe it. I have been paying out of pocket this whole time. My insurance will kick in Nov 1. The Dr I currently see does not accept my new insurance, so I will be going to a different family Dr. I will assume my new Dr will want another blood panel to see where I'm at. If I were to take Ralox to help with the gyno, would it show anything weird on my blood test? I'm not sure where to go with this. Any help would be appreciated.

probably should take some nolva, will get rid of that gyno asap and wont skew your blood tests as far as estrogen is concerned.

Get a physical exam done to make sure it really is gyno. Don't treat this on your own.. Could be other things.

Yes, testosterone replacement therapy (TRT) can cause gyno. Test aromatizes into estradiol which if left too high for too long can cause gyno.

Get your estradiol (E2) checked. See if it needs to be managed. Do all of this asap. Catching this early improves your treatment outcomes.

^^^not much to add after that

I appreciate the input. Last time I had this I had a mamogram. It showed as gyno, no cancer. After the last results came in, my Dr at the time didn't prescribe anything. Just said it was probably form the alieve I was taking. Said it should get better on it's own. I talked to another Dr about it, said it's usually from heavy pot use. I've never smoked it in my life. I just feel like everywhere I turn, there's an idiot Dr. Just not sure why it would flare up off cycle as well.

High estrogen levels cause gyno. See an endocrinologist. It can be treated with medication if early enough. Surgery may be needed.

Gyno won't kill you or anything. But I don't know too many guys that like having a set of C cups and shopping at Victoria's Secret for themselves -- not that there is anything wrong with that if you do enjoy that.

Ok, here's an update. I went ahead and switched to my new GP, I will have when my insurance kicks in Nov 1. I saw him this past Tuesday. I explained about the lump and that I would like my estradiol tested. He said he was unfamiliar with this and would need to do some research first. He did order another testosterone test. He called me Thursday with the results. He said he did some research and I am going in today to test for estradiol, HCG, and couple other things. I will post those results when I can. I at least feel like I'm moving in the right direction. I was disappointed in my TT result though. It was however done 8 days since my last 100mg/wk injection.

TSH 2.152 0.358 - 3.740 uIU/mL
T4 FREE 0.92 0.76 - 1.46 ng/dL
TESTOSTERONE 341 241 - 827 ng/dL

You're still getting testosterone replacement therapy (TRT) dialed in at 9wks in but looks like a small Aromatase inhibitor (AI) dose can help lower that. As ASAP said, ralox at 60mg daily for 10days then 30mg daily until gone after that or nolva 40mg first 2wks and 20mg daily until gone after that will help get rid of gyno

8 days after injection means nothing. This blood work means nothing lol. Your levels are probably 800-1000 if you test them 3 days after injection.
Gyno aside, how do you feel???

What do you mean? It is common practice to check your trough levels.

I do not feel as well as I did a few weeks ago. Kinda like the effects are wearing off. I got my last lab yesterday. My LH was 0.1. Hopefully my doc will get a game plan together next week.

Your LH and FSH are supposed to go to zero while taking exogenous test. Your natty shuts down. Your pituitary is telling your testicles to not make any more test because your body already has enough by not sending a signal.

Check your estradiol.

It was. 39.0 *0 - 39.8pg/ml range

The first few weeks you felt good because your body was not only getting exogenous testosterone, but still making it's own testosterone...thus giving you a big boost. Now your body is adjusting. The big boost at the beginning may have been enough for you to start having gyno symptoms. I would use the tried and true way of combating estrogen..... Anastrozole and Novladex. The anastrozole will prevent the conversion to estrogen while the novladex will fill the estrogen receptors. Kind of like plugging a leak (anastrozole) and then mopping up the floor (Novladex) I know...I am old school....
