High FSH and LH but low total T

Here is my lab test and my Endocrinologist doesnt think I have Hypogonadism or any other hormonal imbalance.

I am 24 yrs male, 98 kg and 177cm

Estradiol 43.5 pg/ml (0.0 - 45.0)
SHBG 53.2 nmol/l (18.3 - 54.1)
FSH 4.8 mIE/ml (0.0 - 6-0)
LH 10.9 mIE/ml (1.7 - 8.6)
Total Testesterone 4.15 ng/ml (2.5 - 8.3)
DHT 778.8 pg/ml (250 - 990)
Free Androgenindex 27.1 (15.0 - 95.0)

Is my T-level normal for my age? I know I'm obese, but if I cut 25+ kgs is there a chance of naturally boosting T-level to at least 7 ng/ml?
 
You're aromatizing a bit of your testosterone into estradiol. That's part of your problem. You're not technically hypogonadal, but judging by the volume in which your pituitary is SCREAMING, it may not be long. Yes, losing body fat will likely help.

What time of the day was your blood work drawn?

What was your TSH at? How about your Vitamin D?

Losing body fat will help. The hard part is doing it when you are struggling with lowish testosterone, but it can be done with a good diet.

Have you had a sleep study done to look at issues such as sleep apnea?

Your TT is a little low for your age. You should keep an eye on it. You did the right thing in getting checked.

How do I reverse that? ie aromatisation of estradiol to testesterone? Can you explain a bit how and why is my pituitary screaming?

My blood was taken by 4pm


My TSH basal extremely high, 4.1mlU/l (0.27 - 2.5). My doctor prescribed me euthyrox for Hypothyroidism. Do you think that might have anything to do with my low T?


My Vitamin D level is also pretty low too 13.8 nmol/l (13.2 - 120) Does that also contributes to low T?


I sleep averagely 6-7 hrs daily.


What kind of diet and exercises would I need to simultaneously lose weight and increase my T level?

First of all, you always need to test your natural testosterone/Hormone levels first thing in the morning. You should disregard your lab results as they were so late in the day. 8AM is best.

Hypothyroidism can lower your testosterone levels and affect you SHBG which will result in less free testosterone. Treating your condition should help your T.

Low Vitamin D can cause low T as well. I recommend more sunlight exposure and supplementing. 10,000iu of vitamin D daily. Reassess in about 3 months with blood work.

The Aromatase enzyme converts test into estrogen. Fat cells contain high amount of this enzyme. Reducing body fat will reduce Aromatase.

Good exercise are strength training with free weights. Compounds lifts like bench press, Squats, deadlifted, etc.

Diet is of utmost importance. You should consider getting the help of a diet coach.
