Low testosterone and don’t know what to do

@novaman
As far as I and my wife are aware I don't have sleep apnea and I also don't have the problems, symptoms that according to several sites are a sign of sleep apnea. Lyme disease has been ruled out and I haven't seen much porn in years.

@juced_porkchop
I am looking for another doc and have formally applied for a second opinion.

Formally applied to who?

Ask your doctor to refer you to a specialist (urologist or endocrinologist)
You should have more joy with them, they deal with this daily...

Talked to another doctor and I was told that there is nothing wrong with 10.4 nmol/l = 299 ng/dl and that problems where perhaps of another nature. They do not treat (young) men with TRT if their testosterone is higher than 7 nmol/l = 201 ng/dl

However he did do another blood test looking at my FSH, LH, Cortisol, testosterone and other things. He did not want to look at my SHBG, Prolactin and Estradiol.

I will get the results within a week.

Sanipalo

Something tells me that hospital you applied for the second opinion too won't do squat for you.
looks like you are in the range by a couple nmol/l.
He SHOULD look at SHBG, Estradiol and Prolactin.
That's the problem with docs who don't understand TRT.
They need to be avoided at all costs.
You need to look at your Bio Available Testosterone and Free Testosterone.

He did the Test in the office?
Test needs to be done in the a.m. preferably before 9 A.M. and after a good nights sleep. Meaning dont go to bed late and put the alram clock on to go for bloodwork if u had 5 hours of sleep.

No not in the office! I had to go to the hospital lab the next day between 8 to 9 AM and they took the blood. I should have an answer within a week.

Sadly no SHBG, Estradiol and Prolactin. I will know more within a week.

Got my second blood test back

First blood test was at 10:00 am and second at 8:30 am with 2 weeks in between

Testosterone has gone from 299 ng/dl to 339 ng/dl (range 200 to 800)

Albumin has gone from 47 g/l to 45 g/l (range 35 to 45)

RDW has gone from 12.5% to 12.4% (range 12 to 15)

Blood platelet count has gone from 178 to 158 (range 150 to 400)

FSH is 5.7 (range 1.4 to 10.6)

LH is 3.3 (range 1.2 to 7.8)

Cortisol is 390 nmol/l ( range 150 to 700)

According to the doctor there is absolutely nothing wrong based on the blood test. TRT is out of the question.

Now I still don't know what is causing my problems.

10 AM is a bit late for blood work on Endogenous T.
Your FSH is highish...

Have you had a sleep study done ?

No sleep study. Although I have used a sleep app to see when I wake up, how much noise I make and how much sleep I get and I could not find anything special.

FSH is within range, what makes it highish?

Your thyroid is a little sluggish, this can cause lowish T. If you optimize thyroid and lifestyle you might be able to get to 700 on an AM draw. Make sure you are fasted and get your testosterone drawn as early as possible at the same time.

I have found that it is better to talk to the doctor and get them down off the high horse so you can actively work with themin your treatment . I've gone threw a few doctors 2 who told me to find another doctor after pressing them for more care for anti estrogens or HCG. The thyroid issues are far more serious then the low testosterone. The thyroid regulates your metabolism. I'm hypo thyroid and when my thyroid shut off I felt like some just unplugged me. I had no energy at all. The doctors are worse at treating it than they are at treating low testosterone.

I'd address the thyroid first then go from there. If you have the choice of armor thyroid choose that over synthroid

Consider treating it.

I talked about this with my doctor. My TSH is 3.15 and the range is 0.4 to 4.0 in my country and 0.5 to 5.0 in the USA.

Because it is within the norm, they see no point in treating it or looking at T3, T4 and other related blood work.

I already get 150 mcg extra iodine a day with my multivitamins and probably another 50 to 150 mcg a day from food. The RDA is 150 mcg. I could trying going up to 500 mcg for a few weeks to see if it help.

Read this and do more research. You will be able to show your docs what the new guidelines are.

Does Your Doctor Know About the New TSH Lab Standards?

Before you jump the gun with iodine....
Start taking your average daily temperature (oral mercury thermometer under the tongue) first thing before you get out of bed each day. Get a series of these done over a week or two and then get the average.
if you are below 97.3 F daily or lower it is a problem indicator. The 150 mcg RDA is not usefull in this case. That amount is suitable for someone that has good existing levels of iodine.
so lets say one has to take in 0.75 grams of iodine for IR. How many days would it take to intake 0.75 grams of iodine with the RDA at 150 mcg?
simple, 0.75 grams/0.00015 grams per day= 5000 days.
But over 5000 days you would lose most of that and for all intents and purposes the RDA will never fix ID.

For IR you can take up to 50 mg iodine per day and get 0.75 grams in two weeks.
