Low testosterone and low vitamin D

Hello everyone, a few months ago I had some blood work done by my main doctor because I had all the signs of low Testosterone.


Here are my results from the first lab.

Tot Testosterone,Male 272 NG/DL 276 - 957 NG/DL <- range
Sex Hormone Binding Globulin 31.8 NMOL/L 14 - 93 NMOL/L
Free Testosterone 54 PG/ML 43 - 227 PG/ML
% Free Testosterone 19.9 % %

I'm 31 and my doctor said my results where low but acted like it was not a big deal, but wanted to send me to a specialist. I had my 2nd lab a few weeks ago and the results are:

Tot Testosterone,Male 408 NG/DL 276 - 957 NG/DL <-range
Sex Hormone Binding Globulin 39.5 NMOL/L 14 - 93 NMOL/L
Free Testosterone 74 PG/ML 43 - 227 PG/ML
% Free Testosterone 18.1 % %

I also had them test my vitamin D because I work from home and don't really get a lot of time in the sun due to my work schedule. I knew it was going to be lower then normal but surprised it was so low.
Vit D 25 Hydroxy 10 NG/ML 30 - 100 NG/ML <-range
I have already started to take around 5000 IU's of vitamin d3 from a OTC supplement.

When I went to visit the specialist he also acted like my original test level was not a big deal, but ran a 2nd test anyways since it was morning he said my level would be higher and it was a little higher but I still feel like this is very low for my age.
It has been 3 weeks and I have my lab results but he has not even called to go over the results?

Should I find different doctors? I have a friend with low testosterone and he recommended I post on here if I had any questions. He said he also had to see 4 or 5 doctors before he could get anyone to prescribe him trt.

Do you all think 408 is still to low for someone that's 31 and in other wise good health?

Your second doc was correct to check your blood work in the morning. That is when your testosterone is at its peak.

408ng/dl is below average for our age, but most docs won't treat you for Hypogonadism until it is below 300. Or below 350 when also combined with low Free T.

Low Vitamin D can affect your testosterone levels. So give that a chance to help. But I suspect you may need more than 5000iu per day to get into the upper quartile of the normal range.

Your height and weight worry me. Are you scrawny? Are you severely restricting calories? Dieting for a long time.reduces testosterone levels. It can affect your thyroid too.

Do you lift weights and/or play sports? Get much exercise in general?

What is your diet like?

How much sleep so you get per night? Any issues like sleep apnea?

I think you should run additional blood work. But I will cover that when you answer the above questions first.
 
I'm pretty scrawny but iv been like that my whole life. I'v always had a hard time gaining any weight. I go to the gym about 4 times a week; my work outs are mostly weight training(a lot of upper body) I work out pretty hard and go to failure. I don't really see any improvements from the gym but enjoy it. I don't play any sports and haven't done a lot of cardio for the past couple years.
My diet is decent, I don't eat the greatest all the time but I also don't eat a lot of junk food like chips or cokes. I rarely keep track of my calories but when I have it's normally 2700 to 3000 a day.

I had my thyroid checked from my original labs here are the results for it.

Free T4 1.21 NG/DL 0.80 - 1.76 NG/DL <- standard range
T3 Total 1.06 NG/ML 0.60 - 1.81 NG/ML

On the week days I average about 7hrs of sleep
weekends when i'm not working 8-10 hours
I don't think I have any sleep apnea but never been tested. I'm a pretty hard sleeper and have a hard time waking up in the morning. (never really been a morning person)

How much vitamin D do you think I should take a day? Iv been taking about 5000iu's I do feel a little better but nothing drastic.
 
I doubt you are eating that many calories yet only weigh 170lbs. Regardless try eating 4000 calories per day. Make sure your get 200+ grams of protein per day and 80 grams of fat. The rest can come from carbs.

I take 10,000 iu of vitamin D per day and it gets me to 80. You will have to get blood work to see where you get and adjust your dose as needed.

Try lifting heavy in the 3-5 rep range. See if that helps.

If eating more, fixing your vitamin D deficiency and training heavy doesn't work you may need to consider TRT.
 
Youve had a month to work on the advice given, whats your weight now? hows the diet op?
