2.) Complete and submit Medical History Questionnaire ONLINE
3.) Print Physical Exam Form. For your convenience, you may go to any CVS "Take Care Clinic", WALGREEN'S "Minute Clinic" or have the doctor of your choice administer the required Annual Physical Examination.
4.) Order blood work right from our website. You will be emailed your prescription for blood work within 24 hours and registered in LABCORP'S system, NATIONWIDE. Have your blood work done at the location nearest you. I usually have results within 2-3 days.
