Potential maximus hrt clients....please read carefully
To anyone who is in the process of getting started with maximus Hormone Replacement Therapy (HRT), please read this carefully...
i have posted in a sticky at the top of this forum exactly how to get started with maximus hrt. There is also a bold note on my website that clearly states that the sf-36 form is not the required medical history questionnaire that needs to be completed in order to begin the steps required to begin therapy.
yet i still, continually, get people that are just completing the sf-36 form and sending it in? My instructions to get started could not be any simpler to understand. Either refer to the sticky note at the top of the forum, or simply scroll to the bottom of the website and complete the three required steps necessary to get started on therapy. Thank you.
