Continuing my thread from here: steroidology.com/forum/testosterone-replacement-therapy/673388-low-t-low-shbg-low-e-cold-intolerance-chronic-sinusitis.html

I have been on a dose of 25mg Clomid every day for little more than 4 weeks.

My blood work prior to starting clomid:

Age: 33
TOTAL T: 270.09 ng/dl
FREE T: 5.84 pg/ml
FSH: 5.75 mIu/ml
LH: 4.16 miu/ml
SERUM PROLACTIN LEVEL: 11.39 ng/ml
SERUM ESTRADIOL E2: <10 pg/ml
SHBG: 12.8 nmol/L

After about 4 weeks on clomid:

TESTOSTERONE, TOTAL 424.20 (241.00 - 827.00 ng/dl)

TESTOSTERONE,FREE 11.30 (8.9 - 42.5 pg/mL)

LUTEINIZING HORMONE 8.29 (1.50 - 9.30 mIU/mL)

FOLLICLE STIMULATING HORMONE 15.34 (1.40 - 18.10 mIU/mL)

PROLACTIN 7.37 (2.10 - 17.70 ng/mL)

So does this mean I am primary? Should I try HCG for a weeks to see if it works, or jump straight to TRT? With LH and FSH that high, will HCG have any incremental effect on my T levels?

Your FSH was highish to begin with.
You still on this?
I heard from ppl using clomid to just stay on it for up to 6 months to see the benefits of it.
how dod you feel while on it?
you're SHBG IS low. TRT might not help u.
guys with SHBG like yours tend to not respond to TRT.
who knows...
atleast your free t increased while on clomid.

yes am still on it coz I do have little better erections and energy (could have been placebo). No improvement in mood depression, irritability etc. The thing which has really improved is my acid reflux, which could be due to higher estradiol (I am getting this pulled soon).

How could TRT not help him. His FSH, and LH are sky high, and his Test isn't. That tells me (not that I'm an expert) that his HPTA is being given plenty of fuel to create Test, and it's still not creating all that much. This to me makes him Primary hypogonadal, correct? That would mean TRT Would help him. It would shut him down, but it would give him the Test that he does not have currently. What am I missing (and that's a serious question, not sarcasm)?

He means that bcoz my SHBG is very low, any exogenous T will be flushed from my body very quickly. Hence, low SHBG guys generally have to do subq injections and that too more frequently (EOD).

But I do agree that TRT should help me, even though I have low SHBG. One of the causes of low SHBG is insulin resistance which itself is linked to lowT, so higher T should reduce insulin resistance and increase SHBG. Another cause of low SHBG is low estradiol, which might also increase with TRT.

Ok Good to know. I'm still learning about every facet, and that is something I didn't know. That low SHBG will allow test to clear too quickly. I'm currently at 33 as of last week when I did bloods.
