shoulder problem

Ok so my right shoulder gives me shit. It doesn't exactly dislocate, but it "slips" around.
An example is when i do upright rows...its fine as long as i keep the weight close to my body, but if i lift anything that way and move it away from my body my shoulder kind of drops and you wont see me doing any heavy lifting or direct shoulder work that week.
Asking if anybody has any suggestions? Remarks? Thoughts?
i drink glucosamine/chondroitin like its water these days and it seems to help a decent amount...but still its like my shoulder just doesn't sit right?
any suggestions are helpful? am i just destined to have shoulder surgery one day? haha
 
I do this twice a week to "try" to keep my shoulders healthy. Upright rows put alot of stress on the shoulder.

DB Retractions 2x12
DB Cuban Rotations 2x12
Band External Rotations 2x12
Band Internal Rotations 2x12
Band Pull Aparts 2x12
Band Dislocates 2x12
Barbell Protractions 2x12
Posterior Capsule Stretch

I don't do upright rows often...
And I noticed the pain and "sliding" after my dumbass weight lifting partner back in my freshman hs football days wasn't paying attention to me as I was maxing out on incline bench press...right side gave out...

Interesting... I have a pretty similar feeling with my right shoulder, but it seems more focused on heavier chest days. The ligaments just seem to pop and move more than the left. The right side def gives out sooner than the left.
 
Yea it blows...there's no such thing as heavy shoulder day for me...my shoulder just won't take it...
And only DBells for my shoulder work...machines, hammer strength, and barbells put my shoulder in an unwanted position.
