25 Y/O - Will TRT cure ED? Need help.

Hey all,

Just came across this forum and I wish I had known earlier. Anyways, I've been suffering from ED for 1 year and would like some opinions on what I should do.

24, 178 cm, 63 Kg.

Symptoms:
Weak morning erections
No erection without porn
Decreased sex drive
Premature ejaculation
Decrease in penis size

I just got my blood test results:

Total Testosterone - 17.2 nmol/L (11.5 - 32.0)
SHBG - 20 nmol/L (11.0 - 78.0)
E2 - 27 pg/ml (10.0 - 40.0)
Prolactin - 638 mIU/L (40.0 - 450.0)

Thyroid - TSH - 1.6 mIU/L (0.40 - 3.80)

Heart and Lungs are good.

No diabetes, no history of injuries.

Yes, the T is low but the GP said it's good. I'll have to try another doctor.

Is high Prolactin causing low T and ED? One of my mate said Cabergoline (Dostinex) will lower Prolactin and significantly boost your overall T levels. He went from 22.7 nmol to 25.5 nmol. But since it's a on prescription it's only temporary from my understanding.

Should I consider TRT as 17.2 is too low for my age as I'm sure everyone knows.

Any help would be appreciated.

Your testosterone is perfectly normal for your age. You are at 500ng/dl. That's right about where a 24 year old male should be at.

You issue is pretty obvious. You have high prolactin. High prolactin causes ED. You need to probably first get an MRI and try to determine what is causing your high prolactin
It may be a prolactinoma. Don't just treat it with a medication. Try to learn what is causing the problem.

@Megatron28 - Thanks for the reply mate. @jojo25jj as well.

500 ng is equivalent of a guy who is 45-50 years old man. Someone who is 24-25 should've over 700 ng. Considering 680ng is average.

Yes, I'm going to do the MRI test and treat high prolactin level. But would you guys recommend TRT? I could easily get Testosterone Enanthate from overseas if my Doc here doesn't allow. Few people who went from 400 to 900 ng said the difference is night and day.

Don't think TRT is helpful for you. Your levels could be ok for you... you should test Dostinex at first.
I would think this alone should be helpful! :)
With TRT you will have evtl. more issues to counteract (E2) what is also not so easy.

Your numbers are not correct. A 45-50 year old male actually has higher Total Testosterone levels than a 24 year old male. However his Free Testosterone is usually lower. The average for all males is around 600ng/dl. Testosterone for Hypogonadism typically doesn't get prescribed by doctors until Total Testosterone is below 300ng/dl. I am pretty confident Testosterone is not the cause of your ED. The cause is likely your high prolactin.

Click the FAQS link below in my signature. It has a chart included in it that shows average TT and FT levels by age.

Make sure you get your diet in order, lift weights, exercise and get plenty of sleep. Those things can help improve your TT levels.

I'm 26 my tt is around 400 to 450 and while I don't have many morning erections I can get it up with porn or a girl it's not hard or easy but normal i also have a varicocele(getting surgery next month) go to your urologist make sure you don't have one could give you a tad bump in tt...on another note man don't put so much into those ranges under 350 is where it hurts I hear...and if your stressing it will make it worse...i had ed real bad while stressing about this varicocele then I started relaxing scheduled surgery got back to working out and guess what got a mean bj from a chick and splatted all over her now I still think I need time for my levels to raise higher but what I'm saying is relax don't pressure yourself into getting hard it will make it worse and sleep

If you have bad cholesterol that can give you ED, and if your PRL is significantly high then you probably have a pituitary adenoma.

Hi, I noticed you said no erections without porn? Porn ED is very serious these days I know people who have had this. I know this sounds hard but try and quit masterbation and watching porn for 2 weeks. No porn at all. See if anything changes. Let me know :)
Like a few have said 500 tt is actually okay so this shouldn't be the issue give what I said a try :)

I got a varicocele waiting to get operated on I've lost morning and random erections I think it's impacting my test. I'm getting it fixed but I think I'll end up on trt anyway

BUMP.

Hey guys. So my GP ordered a Brain MRI (he didn't mention prolactin elevated levels) and it was done without injecting contrast. Not sure if that makes a significant difference. I just got my MRI results back. Everything is normal. No evidence of Prolactinoma.

Now he says there is no need for further treatment. The elevated levels could be due to stress or other medications lol. This is the same guy who said T3 is the same as FT3 when I did Thyroid blood test. After forcing him to prescribe me a medication he ordered another blood test just to be sure. It's tomorrow.

From my understanding, Cabergoline is the best medication for lowering prolactin. I could get it from overseas if this idiot doesn't prescribe me. I would like to get your opinions if I should take it or there is alternative?

Cheers.

Why wasn't Free Testosterone checked, TT is one thing but if your bioavailable testosterone is low it don't matter where your TT is. Stupid doctors!

What other medications are you taking?

What other medications are you taking?

This is a very important Q'. I myself have had high prolactin and ED. the prolactin causing no orgasm. some of my cause was due to my psychotropic medication.

I use caber for the prolactin and Cialis for ED mostly cause by my AAS usage running havoc with E2. I keep my E2 in check and the ED is not bad however I am 66 years old and need a little Cialis just naturally.

As stated you need more investigation MRI's and the such. Prolactin caused by brain function.

SO ergo tell us about any and all meds you are taking.

Good Luck

@megatron28 and @oldmusclemike - Once again thanks for the reply.

I'm not taking any meds. My blood work -Thyroid/Full blood count/ Diabetes and everything else including MRI is fine. So yeah. The symptoms I have - chronic fatigue, Low T and ED are presumably caused by high prolactin?

So should I just tell my Doc to prescribe Caber if the levels are elevated again? I honestly don't know what he is going to say. Docs in Australia need to be forced in order to get them to take you seriously.

First of all, you don't have low T. We discussed that earlier in this thread.

You said your doc thinks the high prolactin may be due to other medications you are taking. Why would he say that if you are not taking anything?

it is the elevated prolactin that is causing ED issues. Must find out why your prolactin is elevated. Good to hear they are trying to find the source

High Prolactin decreases T production. We know that. Having read about Prolactin, once it returns to normal the T levels also return to normal. From a study, patients increased their Total T by up to 35%. Chronic fatigue is one symptom that has been bothering me more than ED. You can't do anything with low energy levels. I think it's safe to say the elevated prolactin has affected my T levels. Every other blood test is fine. I'm afraid I must disagree about the low T thingy. Here I'll attach a Total T graph by age.

As for my Doc - He said it could be stress or medication (I told him I don't take anything to which he ordered another test). You've to remember I forced the Doc for Hormone Blood test. Trust me when I say this, the GP's here don't know much.

[A 45-50 year old male actually has higher Total Testosterone levels than a 24 year old male]

I'm sorry, but no study agrees with this. Refer to the graph above. I can cite studies if you would like. It is well known Total T levels start to decline after a certain age (30 Y/O). 60 Y/O old men in 80's had on average over 550-600 TT levels which have been declining (less than 450 ng for instance in 2002 study). If you think a 25 Y/O with 500 ng is normal I don't know what to say. Either way this is another topic for another time.

I'm here for second opinion on how to lower Prolactin. Since there is no prolactinoma I'm not sure he will prescribe Caber.

Here are the charts Megatron is talking about with total T and free T

View attachment 566856

You can see for Total T at 25 you might be as high as 1,050 and average is 600 ish. At 450 you are below average, but not low.

Free T might be as high as 15 and average 8 ish

You need to repeat the blood test and get total T, free T, E2, and prolactin at a minimum. If your prolactin is still high and doc won't prescribe, then try black market caber.

Also If I did the conversions right you are 5'10" and 139 lbs. That has to be pretty weak. Get yourself into a gym and do some weightlifting, that will help your overall state and help normalize your hormone levels. Chronic fatigue could be due to bad diet and lack of physical activity, and/or vitamin deficiencies.

You should be fixing diet, trying to workout and gain some muscle, before running down the road of medical treatments. This only gets worse as you get older, so you'd better beat this now when you are young.
