I think the guy has a disease with the lower extremities...I used to run with a group of people and we had a guy that had that lower feature look but it was more bony and tiny..He had no problem with running......I never asked what the guy had. It's some sort of imbalanced look...I don't think it has anything to do with not training the legs.You would think the lower legs would be a little bigger because of his mass look and the default look of the lower legs would be at least be a little bigger.
My legs look much worse than the guy in the pic and I never skip leg day, I have no problem building quads but I can't seem to hit calfs good enough, they don't even look like they've grown since I started lifting .
