Lots of ED problems lately...

So my doc hesitant to prescribe because his other testosterone replacement therapy (TRT) patients are not on it and he's inexperienced in it, but he did say he's here to "satisfy" me. If I ask him to get me arimidex, will he be scrutinized by medical boards?? Is arimidex a "serious" drug?

All drugs are "serious". It is not a class 3 drug though. However, you are asking him to prescribe a drug for "off label" use. This is common with drugs, but it can make some docs uneasy.

Tell him to read - and get you a copy-- of the New England Journal of Medicine study on hypogonadism and aromatase inhibitors that came out last week. That might help put him more at ease. Or ask him to refer you to an endocrinologist. My GP wouldn't prescribe an Aromatase inhibitor (AI) for me eithr because he wasn't familiar with them. So he sent me to an endo he works with. He gave it to me.

Hmm. How is hypogonadism related to testosterone replacement therapy (TRT)? My doc is treating me for hypo-testosterone. At first I thought I have HIV, but that's not the case... So they're not sure what is causing my low test. Is it hypogonadism?

Also... I can't afford an endo right now. And I told my doc, and he said something along the lines of "I'll work with you." Can regular docs write AI?

Is there something I can print out and give to my doc that can maybe help him understand that Aromatase inhibitor (AI) is often used with testosterone replacement therapy (TRT)? (So that he doesn't have to resort to referring me to Endo? He's a really nice guy and he says he'll help me whatever he can)

Hmmmmmmmm. Can you look up hypogonism on the world wide web and give me your thoughts on how it might relate to testosterone replacement therapy (TRT)? What do you think?

Anyone with an MD should be able to write a drug prescription, including Aromatase Inhibitors.

Just ask you doc how he thinks you should deal with high estradiol. Certainly it can't be good to be above normal, right? If he needs prodding, tell him your research shows that you need to stop the aromatase enyzymes from aromatizing testosterone into estradiol. Now go use the internet to learn more about aromatase enzymes and aromatization. Or go back and read what I and others have already told you about the process. :-) And do you recall that New England Journal of Medicine study last week that I recommended to you earlier today?

Maybe print all these posts out and take some notes. That might help you remember earlier conversations that you have had.

Have you read the stickies in the testosterone replacement therapy (TRT) section yet?

I can't remember. How old are you?

Didn't question your authority on this matter. Your response has always been very informative. I'm 25. But I'm worried that he's going to be like, the things on Steroidology is BS. How do I get a hold of that New England article?

I know you aren't questioning me. I'm just trying to get you to listen better and do some homework on your side. You keep asking the same questions over and over again and it seems like you are not trying to research anything on your own.

- have you read the stickies yet?
- have your read the two links I gave you yet?
- have you looked up hypogonadism yet?
- have you read about aromatase and aromatization yet?
- have you re-read everything again?
- are you using the search capabilities of this website?

You need to know this stuff BETTER than your doctor does so that you can help guide him to treating you properly. And trust me, it won't be very hard to know more than your doctor does when it comes to testosterone replacement therapy (TRT). But you need to be mature and do the homework. Time to hit the books again.

As I previously suggested, ask your doctor for a copy of the study.

Won't he be offended if I know the stuff better than he does? Also can you show me link or briefly touch on how high estradiol can cause acne? My doctor is worried that if he prescribes Aromatase inhibitor (AI) for me, it will make my acne worse.

If he is offended, then he is the wrong doctor for you. Do you get offended if someone knows more than you do on a topic you are knowledgeable about?

Here's a link. He is thinking about estrogen's effect on women with acne. Women with acne generally benefit from increasing their E2. For example, taking the "pill" is known to reduce acne in women. Tell him you don't have ovaries. :-)

Serum hormone levels in men with severe - PubMed Mobile

By the way. Don't press the Aromatase inhibitor (AI) issue because you have acne. You are missing the boat. Tell him you have labs showing you have high E2 and you DON'T want to get gynecomastia. I doubt a set of double D's would like good on you.

Actually... I'm starting to see some gyno... Is he gonna make me take off my shirt to show to him lol. That would be embarrassing.

Will he just leave my high E2 untreated though? You mentioned something about lowering my test... If he decides to do that, what can I say to him that would make his think twice? I'm at about 850, but I'm not where I want to be in terms of fatigue, etc... if he lowers it, I would be back in square 1. Currently i'm at 1.5cc/2 weeks 200mg / ml . I shoot up twice a week so about 0.37 cc / time.

When I was at 1cc/2 week I was at 380 test.

There you go. You already have your answer. Simply say you like how you feel at the higher TT and you want to try using an Aromatase inhibitor (AI) to manage E2. Don't over think this stuff. Be prepared, but don't get all crazy.

Just thinking outside the box here but if worse comes to worse you could always order an Aromatase inhibitor (AI) from one of the site sponsors and treat the high E2 yourself.

Google for new england journal of medicine, then type in the search box hypogonadism, then sort the articles by date and choose the one that is from one week ago(september 12th). I think it's not that hard, especially for a 25 year old.

While your at the doc's - see if he can prescribe you something to help you stop saying 'Hmmm'

It's more than annoying!

Good luck with the other stuff though :)

I couldn't ever find one that had the full study. I haven't tried recently though. I had a friend email it to me, but the file is too big to post on this website. Let us know of you find a link to the full study.
