40 yr old male Started TRT at the end of the summer doing 200 mg Test cyp/wk ( 1 injection per wk) taking 250 iu HCG every 3.5 days and .5 mg ADEX every 3.5 days on the same days together.

Diet is clean but not strict. Take lots of fish oil and vitamin D.

about 5'11 and just under 200 lbs with approx 20% BF

A few things are flagged here.

AST (SGOT) 45 High 0-40 (No clue what this is but its high)
ALT (SGPT) 34 0 - 44

Lipid Panel'

Cholesterol Total 170 100 -199
Triglycerides 70 0 -149
VLDL cholesterol cal 14 5 -40
LDL cholest cal 120 High 0 -99 ( Too much fish oil??)

Thyroid panel

Thyroxine T4 8.1 4.5 - 12.0
T3 uptake 35 24 - 39
Free Thyroxine index 2.8 1.2 - 4.9

LH .1 Low 1.7 - 8.6
FSH <0.2 Low 1.5 -12.4


Test serum 935
Free Test (direct) 33.6 High 6.8 - 21.5 ( Whats up with this?)

Estradiol 25.7 7.6-42.6

IGF 1 is at 325 high 69 - 226

AST and AST are your liver enzymes. An elevated AST likely means you have been on anti-inflammatory such as tylenol or have been drinking alcohol in close proximity to the test. Of course there are other reasons why this could happen, but as you're barely out of range, I'd just hop on 500-1000mg of NAC every day. That'll clear it right up unless you're dealing with something serious (doubtful, but do keep an eye on it in the future)

LDL is "bad" cholesterol, and is not typically raised by consuming fish oil. In fact, fish oil should lower your LDL cholesterol. I'd look into picking up some garlic or adding garlic to your diet. If you consume vegetable oil or cook with it, I would look into changing that out for olive oil. That should help significantly.

LH/FSH SHOULD be low. That's part of your negative feedback loop and is working normally given the fact you're on exogenous testosterone. I'd worry if they weren't that low to be honest. ;)

As long as your doc is fine (assuming this is prescribed) with a slightly higher Free Test, there is nothing to worry about and in fact, that's a good number. Free testosterone is the part of your total testosterone that is actually unbound and able to do work in your body such as regulating metabolism, building muscle, and all the other positive effects the hormone carries with it. This is typically higher than normal if you have a lower SHBG, which is fine as well - provided it's not rock-bottom low.

Nice estradiol. Are you taking an Aromatase inhibitor (AI), or this without? Looks like your protocol is dialed in nicely, I'd give it an A-. :) (minus due to LDL and AST, which are both easily remedied.)

Yes Im taking .5 mg of ADEX every 3.5 days, the original script was EOD and after reading on the forum I didnt want to crash so I changed it around a bit ( not sure if my DR will like to hear that! But hey , I was right!)

I do take the occasional advil now and then for when my back gets all stiffed up on me. And I do have an occasional beer or two with dinner. I have had elevated enzyms in the past too. Had an ultra sound that was negative.

As far as the LDL Im a bit pissed cause I really do try hard to eat clean. I cook with olive oil and coconut oil only. I use the stuff generously. Pretty much cut dairy out and will only eat maybe 2 yolks at the most . I will try the garlic, hope that works cause I aint going on anti cholesterol !

I would talk to your doc about dropping the adex, you may not need it. That could fix chol, liver and free T issues.

Although olive oil is extremely healthy for you, cooking with it, especially at higher temperatures is actually unhealthy. Just my $.02. :)
 
you could drop the a-dex as suggested. Be sure to get an estradiol sensitive panel.
Most importantly how do you feel?
If you're able to. it's better to split up your injection into 2 100 mgs injections. Doing this will drop your estrogen a bit and might reduce the need for an Aromatase inhibitor (AI) altogether. IMO your liver enzymes are elevated because of the arimidex, and possibly the cholesterol as well.
PS: egg yolks are fine :)

Where are the rest of your labs? I have never seen a lipid panel without HDL. Or a thyroid panel without TSH.

Did you get a CBC or Metabolic Panel?

I agree on trying to lower your Aromatase inhibitor (AI). It looks like you got the regular estradiol assay rather than sensitive. Regular often comes in about 10 points lower than sensitive. That means you might be at 15 which starts putting too low. Maybe try .25 every 3.5 days and see how you feel.

It's when you burn it, not just high temps.

Regardless of increasing his HDL it would also most likely increase his already high LDL which isn't advisable.

The research on Coconut oil increasing LDL are not as conclusive as they are on Coconut oil increasing HDL. Although I personally like coconut oil, i think like all things it needs to be consumed in moderation. The greater variety of healthy fats, the better. Just my $.02.

Missed the HDL....

HDL is 36 which is Low. Range is > 39.

Also there isnt any TSH (wtf?) I will ask the Dr about that. Maybe I gotta take another BW. (thanks for catching that Megatron).

And it is the regular E2 test. so you guys are right on that....Im gonna lower the Aromatase inhibitor (AI) and work toward dropping it. I have been considering splittting my dose of test up too.

I did get the CBC done. I didnt put in the post cause its all within range...no flags.

I always cook using a low temp. Coconut oil for my eggs every morning. I will switch back to olive oil though.

Are you donating blood regularly? How high is your hematocrit?

I am not but intend to. Hemocrit...... I didnt get that checked.

Also I missed IGF 1 is at 325 high 69 - 226 which from some of the limited reading I have been doing is not necessarily so bad. What do you guys think?

Why not try canola oil? Its low in saturated fat, high in omega 3-6, and helps lower LDL similar to olive oil. Its smoke point is higher (425°F - 475°F) than olive oil (325°F - 375°F). It also has a neutral flavor. I find frying with olive oil gives food a distinct taste, nothing horrible but I don't prefer it.
 
You said you had a CBC. Hematocrit is part of that panel. If it isn't in there, what was your hemoglobin and RBC?

RBC is 5.57 4.14 - 5.8

Hemoglobin 17 12.6 -17.7 very close to being too high!!

Hematocrit is 49.6 37.5 - 51 Looking high! Gettin there.
