Very very low testosterone

Hello everyone , hope all you fellas are doing great . Guys I recently got my blood work done and found out that I am having really low level of test(176) but before that I have put up my previous results with date and a brief history .

TT 492
E2 55

After here I started tamoxifen 40 mg ED because I felt my already existing gyno was getting bad so I thought of trying it because I read that it binds with the estrogen receptors in the nipple area .
After probably taking it for probably 20 days and got the tests done again and my result came out to be

1 June 2017
TT : 828
E2 : 81

After this I felt more crappy , I posted here too about this and after discussing with a few of the experienced people I decided to start with 0.25 mg adex . EOD with 60 mgs Ralox ED

I stopped both on September 20 th and got my test done 3 days back and got my tests done again and they were like this :-

TT 176
E2 30

I Don't know what has made my TT crash so much , it used to be on the lower side always because of my past cycles but has never been this low . I am 30 and I haven't done any cycle since 2011 .

I have tried clomid 25 mg EOD in the past and it has worked to increase my TT . So should I start with clomid again . May be Clomid , adex and ralox ? What are your suggestions ?

@casanovaaa bro I started cycles at an early age with very bad knowledge . Someone guided me and I followed . Years after when I started reading I got to know about pct's . So you can imagine I did cycles without pct. which I believe is the main reason for all this . No blood tests before or after , nothing .

wow, well no use crying over spilled milk. I hope you can get the proper info here so you can get back to feeling good again. and i also hope this person who guided you with your cycles is no longer your acquaintance.

Definitely not, he was a coach at a gym where I worked out , I was in college back then and I also don't want to blame him but I feel I am the one responsible for it more because of my ignorance to start something that I knew nothing about . I believed someone without asking him about the stuff .
As I grew older I started reading and and I came to know about the precautions I should have taken .
 
When I saw your numbers, I thought the high test was the anomaly; not the low test. The low number unfortunately is probably your 'normal '

Are you doing the sensitive/LCMS tests? I always worry about interferences with the cheaper test methods.

You gotta get that E2 under control or no gyno treatment will be effective! Difficult since you don't have a history of AI use to guesstimate from. That's gonna take time and Labs. Unfortunately.

No bro my testicles aren't shrunken . Also as I said I have used clomid in the past(without being on any cycle ) and my TT increased quite a bit after that , doesn't that mean the blockage is somewhere in the brain( HPTA) ?
 
Do full blood panel including lh and fsh, what about your testicles?Are they shrinked?

Yes, to me that is exactly what it means. Your balls are capable of doing their job, but they aren't getting the signals to do so. Could be hypothalamus GnRH, or could be pituitary LH & FSH.

For testing at a minimum you want LH, FSH, test (preferably LC-MS), estrogen (preferably sensitive) - and then optional but would give you better info free test, SHBG, prolactin.

Seems to me like you might want to try an HPTA restart protocol. There are several out there on the web you can research, they tend to involve both clomid and nolva, and most start with a round of HCG. In your case the HCG might not be needed as it seems your balls are working ok.

You mean LH, FSH and TT all 3 LC-MS ? Most probably where I am from they won't have it , they didn't even know about Sensitive E2 .

Are there any posts about HPTA restart protocol on this forum ?

No, just testosterone total LC-MS. LH and FSH are bog standard tests you will get with any typical profile.
 
OP go see a endocrinologist we can only help so much here but any way you split it this situation ends with you seeing a endo or trt specialist. Don't worry to much about the low T just set up an appointment with a endo and in about 6 weeks after your first shot you will be feeling better.

I appreciate your advice bro and it makes total sense as well but the problem is , I have already been to an endo , long time back . He is one of the best endo's in my area . I am from India btw , the guy did not even discuss about clomid with me, he said we don't even recommend that .

He only asked me to try DHEA's . I posted a post here too after meeting him and before meeting him , infact I went to meet him after someone here asked me to do so .

Are there any posts here on this forum regarding the HPTA restart ? May be I can try that ?

You may need to start PCT, I need to look into that first. Also do you have a full blood test I can see?
