Dude seriously that squat is pathetic, especially benching 400 (if this is true). I promise you have a lot of room to gain in this area. Set a goal for yourself: you won't even consider AAS until you can squat more than you bench.
I know that sometimes there are special situations though, so I apologize if you only have 1 leg.
When I go to heavy I feel like falling forwards onto my face.
Any tips to fix this?
Keep the weight on your heels, deep breath into your stomach, chest up, eyes and head forward or slightly up, pull the bar into your traps, purse the air out at your sticking point, push your knees out as you drive up, and work on heavy ab movements.
Also, forgot to mention get some construction boots/military boots or flat soled shoes. The last thing you want to do is have some running shoes with some real weight on your back. I squat barefoot, but that's only because my gym allows it.
