blood test q?

i have a referal form to a lab and wanted to "add" a few more tests. I am getting my iron checked (tibc) and the following:

"standard"

CBC
ALT
CREA
FERR
GLUF (f - fasting)
LDL (inc. chol, trig, hdl)
tsh (only)

"other tests not listed"
iron, tibc

i am thinking about ticking the electrolytes box (na, k, ci, co2)

and wanted to get a hormone panel done (test, free test shbg, lh, fsh)

i can only see tick boxes for LH and FSH ... how woudl a dr. order that my test and free test levels get checked. i will add that to the "other tests not listed" section.
