fertility issues

so i went and did a semen analysis yesterday since the wife and i have had no luck since summer..

just to update

200mg cyp a week
before nov 1 200iu hcg twice a week.. after nov 1 400iu eod

semen analysis came back 5.5 million.. healthy is 20 million

so here is my quesiton, since spermatogenesis takes 3 months to occur have i not seen the full benefit of my 400iu eod yet??

You might want to start looking into hMG. Hopefully you don't need it, but just in case. And all the other stuff like no Whitey-Tighties.

This. HMG impact FSH more-so than LH (HCG does the opposite). FSH is responsible for spermatogenesis. Id look into it (HMG).

yes im aware of that guys.. just got insurance so i have to go through the loopholes before getting there..

Spermatogenesis takes 90-100 days (average) to occur and can take up to 6 months to see the full benefit.

Despite it only being an LH analogue, the studies are pretty conclusive on HCG's awesomeness at boosting fertility.
Unless you have previous semen analysis results to suggest nothing at all is happening, I'd stick with the HCG for another few months man...might need to consider dropping your TRT dose to 150mg to see if that helps too.

Ill defer to my friend Johnny. Very similar (almost identical as far as treating the problem) & It took him 5 months to get #'s in line and 3 more for he and his wife to conceive. Based on that I would say it is not unreasonable to expect to have to wait another month or 2 for those #'s to creep up to a respectable level within range.

so youre suggesting that a lower trt test dosage would benefit my sperm count?? if thats the case i can drop it to 100 without thinking twice about it

Possibly. Bumping up the HCG dose (if you can) wouldn't hurt either.

i always wondered if there is a curve in the negative feedback loop of the hpta based on trt dosage

you suggested dropping dosage to 150mg.. this suggestion must have be made assuming that the lower dosage might help spermatogenesis..

which in turn assumes that the hpta does not completely shut down at lower dosages or at least is more responsive at lower dosages..

so.. if i were to be at 50mg test a week vs 150mg test a week.. youre suggesting my hpta or at least the hcg being used would be more effective.. hence a curve in response

That suggestion was made because I'm aware of patients regaining fertility once trt dose was lowered & hcg was increased.
I'd have to consult with the folks to find out the mechanisms behind why this works.

hey im already gonna drop my trt dosage as low as possible.. then go mono therapy if i have to.. ive got it planned out fairly well..

its time to have a baby...

The dose of HCG 3J is currently on is pretty good at 400 iu EOD.
500 I.u 3 times a week can be tried..
But I think this needs more time as well as he may need to come off TRT for a bit and just run the HCG mono...

I don't want to get off track here or open a can of worms...but Dr. Scally recommends to his TRT guys to come off TRT every 18 months or so and restart...
I guess in fertility situations like this it's justified...
Provided u are not primary hypogonadal and therefore it's a useless attempt...
