High PSA: I need some advice

I've been on HRT for 9 months. Here's what I take (I'm 66):

1ml testc injected weekly
.5mg anastrozole orally weekly.
500IU HCG injected twice weekly

In the two years before HRT my psa had risen from the 1's to the 3's. It's now at 6. My testosterone is 1323, hematocrit is at 52 and my estradiol is also at 52. A recent digital showed an enlarged prostate, but that was all.

My doc wants me to reduce testc to .7ml weekly to get my test level down, take .5mg anastrozole 3X weekly for the time being to get estradiol down, plus give blood to get the hematocrit down (I did that).

I'm mostly concerned about the high psa, though. I'd appreciate any advice. Thanks.

Definitely donate blood. Do so regularly to control hematocrit.

I agree with lowering your Test dose. That should help, but I would not increase your AI dose. I would leave that the same as you do not want to crash your estradiol. Lowering the test will help bring down your estradiol. Get blood work in about a month and reassess.

Are you taking Cialis? If not, I recommend beginning 5mg daily. That would be a huge help.

Thanks for the advice. Maybe what I'll do is split the difference on the AI and go .5mg 2X per week.

I'm not currently taking Cialis. If it costs anything like the $35/pill I recently paid for Viagra I doubt if I can afford it.

First thing then is to get on daily cialis. You can order it from RUI. It comes in liquid form and is taken orally. Get some 1ml oral syringes too. Cialis ia well documented for improving BPH.

I really would not recommend upping your AI dose if you are going to lower your test dose. One or the other. Not both. You will crash your E2. Trust me. Low E2 is no fun at all.

Edit: Actually you should go to .25mg of Arimidex twice a week. Take it at the same time as you inject your test. Do you inject your test once a week or twice a week?

Where i live i talked to doctors that told me they have ALOT of young men removing there prostate after taking large amounts of testosterone, keeping test below 900 seem very important. Im sure everything will be ok when you got that test down.

"Do you inject your test once a week or twice a week?"

I inject once per week.

Try going to twice a week. It will even out your hormone levels (TT and E2). This may prove helpful as well. And then you could take the AI with each injection.

I just checked the Costco price of 5mg Cialis -- $8.50/pill. Yikes! I doubt my insurance covers it, since they don't cover Viagra. Anyway, I got some good advice here. Thanks.

Like Mega mentioned, you can get the cialis from RUI (banner at top of forum). It's really pretty reasonable, a bottle will go a long way at 5mg/day.

I just started RUI liquid Cialis ("Liquid CIA" on the RUI website). I took a "for fun" dose of 30mg a few days ago and let me tell you, this stuff works VERY well. I took 15mg in the a.m. and another 15mg in the evening. Probably overkill but I wanted to front-load a large dose before I start dosing daily. I did not experience any negative side affects at all. My girlfriend might say different as she was pretty sore the next morning!!

My long term plan is 10mg everyday for prostate health, lower BP and the ability to "perform" on demand. I can't find the thread but I found that the recommendation is 5mg daily for men under 30 and 10mg daily for men over 30. You can do a google search: "daily cialis and TRT" or "liquid Cialis pre workout" etc. A LOT of positive info and I couldn't find anything negative except the cost. Some guys do experience side affects but I did not.

Dosages with RUI Liquid CIA: Bottle is 30mg/ml in a 30ml bottle. They do send you a 1ml graduated oral syringe when you order. Each 0.1ml is 3mg of Cialis. Again, I'm taking 10mg daily and will bump it up to 15mg or 20mg in anticipation of any "fun" activities!! The increased dosage is good for a couple of days. This 30ml bottle should last me 90 days at 10mg per day. So about 20ish dollars a month for me! I think I can afford that. (5mg daily should last damn near 6 months).

If you take a daily dose, you don't need to bump it up for the "weekend". You will already have high enough saturation. You are just wasting it.

need to titrate the T dose down a tad.

Some BPH at those levels is to be expected.

You are the pro IMT... so I'd like to ask; does his PSA of 6 bother you - when the reference range is 0-4?

I'm only asking because I've always read that I need to stop TRT of PSA bumps up rapidly (I'm at 0.4)

Is this an indication of prostate cancer?
Does he need a biopsy or something before continuing Testosterone?

At 66 years is it even a concern, given how slow prostate cancer would normally develop?
How much faster does PC develop if exogenous test is added?

I hate to be the one to bring this up (I can already hear the May1010 jokes) but have you had a DRE? A PSA is only a part of the prostate prognosis. I would recommend a small stature doctor with small hands......just saying....

Usually my PSA is between 2 and 3. But if I have sex the night before, it will be between 5 and 6. That makes doctors nervous. So to keep them happy I avoid sex for 2 nights before blood work and I get good PSA numbers.

You need to see a urologist so you don't miss a silent form of prostate cancer. Testosterone will hasten a prostate cancers growth exponentially. Would do it sooner than later.
