Is a peak testosterone level of 1450 getting dangerous?

I've been on TRT now with injectables for 8 months now. Doc put me on 200 mg test cyp per week which I administer in split dose, and 500 iu HCG 2x week. I also take an AI. Over time I've slowly noticed my peak levels 24 hours after injecting creeping up. The last I checked it was at 1467. The top of the normal labcorp range is 1197. My doc was ok when I topped out at 1250. Am I in a dangerous place now?

Over time perhaps. Nothing acute though. Just keep watching your blood work and donating blood periodically to control hematocrit. Keep an eye on your blood pressure.

How do you feel?

I've been vigilant about donating platelets and occasionally rbc. My hct is at 47.1 last I checked. Can't say I feel any better at higher test levels, but am noticing more body composition changes ( although this could be happening even t lower levels) and a marked increase in frequency of erections

That's a strong dose for TRT. Keep watch of HCT and BP.
Estradiol as well. 1 k HCG a week too.
Wow

Im on Nebido. Been taking my shots every 6-7week and my T is 40nmol at week 2.

Id almost guess that the peak is actually 45nmol at week1 "day 7" and then slowly starts to taper down as the plasma of the undeconate ester slowly releases. Doc, sent a letter and said that my blood count is crawling towards 50% its 49% now... I guess he is talking about RBC?

I have regiously been workingout like a maniac. 2hours everyday 4months in a row, with 1hour cardio added in now and then.. Finishing a documentary about my life, cancer and fitness I started to feel a collapse, tired, drawnout, bad mood, worse sleep, lost weight. So i took my injection at week5, wich eventually NOW has spiked my T even higher. It did help with the tirednenss, but a no brainer would be simple overtraining.......

- Im going for every 7th / 8th week now, and if doc approves ill be splitting the 1000ml into 500ml shots. So that i can level out my levels instead of jacking them up and ramming them down!

Idd say a consistent level of 35-25 and i would be very happy.

He's referring to your hematocrit, it's the measure of how thick your blood is. 50% is getting up there, it's time to donate. 54-55% is the danger zone, don't ever let it get that high. You'll probably need to donate blood quite regularly being on TRT.

Working out for 2hours every day is not a good thing man. You're way over doing it. 1 hour a day 4-5times a week is plenty. Overtraining is worse than undertraining, don't forget that.

I agree with a consistnent 25-35... although personally I'd aim for 30-35n/mol.

So I am still starting to work out the AI dose. I crashed at 2 mg a week then at 1 mg a week, even .5 mg a week is turning out to be too much. Right now I'm going to let the E2 rise a bit as it still is at 15 on the non sensitive test.

I'm just wondering if anyone knows of any documented adverse issues with someone who was on TRT at a 1500 level for a long time

Well this is anecdotal, but after about 2 years at a similar level, I started having problems with an irregular heartbeat, which I never had before. My iron (ferritin) was really low from donating frequently to control my Hct. I was afraid the arrhythmia would disqualify me from donating, but I also felt I couldn't afford to lose more iron but was sitting at %53 - between a rock and a hard place.

I've since gone down to 60mg twice a week and things are looking under control (six months later). This puts me around 900. I feel ok, but I felt better on the higher dose and I've lost some muscle and strength.

Ok I've been on this lower dose for nearly a month and I definitely feel worse. Wife has even commented on it. Lower energy and I tire easily. I'm tempted to go back to the original dose my doc put me on. Any thoughts?

I don't know how many times I read this, but concentrate, please.

TOTAL TESTOSTERONE IS MEANINGLESS.

FREE TESTOSTERONE IS THE KEY.

Your SHBG and estradiol among other things like prolactin, vitamin D, LH, TSH etc., are also of major importance. I don't know about the USA but here in Europe when you ask doc for a blood test, not hey do what is called a "Standard Blood Analysis". This measures ONLY total testosterone and never Free nor does it measure Estradiol.

Now MY doctor says that men don't have any estrogen so why in Gods name would I ask for it. My total T was 484, very high for a 74 year old man he says, and why would a man of my age want an erection?

When my wife asked for Total and free testosterone, he looked at her as though she was insane and told her that testosterone only occurs in men!

Aren't doctors great?

I wouldn't be too worried about your peak level 24 hours after the shot. I'd stick with monitoring your trough. I want my set point to be set by making sure I never go below X. At last check my trough is around 1000-1010. That's 3.5 days after the last injection. I'm assuming that my peak is getting at least as high as yours, maybe not, but I've never checked 24 hours after the injection. Of course as everyone has said, keep your blood work checked periodically, and your BP, and most importantly how you feel.

That is far from the truth. Total Testosterone is very meaningful.

Must be my problem too because I agree. It works (should work) like this:

Symptoms?
  • Check TT, LH, FSH, estradiol.
  • If hypogonadal, and TRT is needed, start therapy. (skipping two steps here to get to the point as we're assuming someone on therapy)

Still problems 90 days into therapy? (arbitrary number of days, just wanted to ensure that TRT has had time to take hold)
  • Check free testosterone, prolactin, vitamin D, SHBG, estradiol, cortisol.
  • Modify protocol as needed.

When I first asked my doctor for blood work, he did a "Standard" blood analysis my TT was 484. He told me that my testosterone was VERY high for a man of 72, it was just before my 73 birth month.

After research and advice on this and other forums, I went back and asked for another blood test, including Free testosterone, estradiol, FSH, SHBG, Prolactin LH etc. he flatly refused and told me that only women have estrogen, not men and there was nothing wrong, in spite of the fact that I was getting fatter, weaker and hadn't had an erection for almost 6 years.

I had to go to another doctor and DEMAND, even threatening them to go above their head, to get my FULL bloodwork. When it came back, my FREE Testosterone was almost nil, my Estradiol was almost nil and my SHBG was up in the clouds. Finally they sent me to a total moron, who called himself an endocrinologist. He told me that I SHOULD be on TRT, but because of my very high total testosterone and my age (I was then 73) he couldn't prescribe anything.

I asked him what I should take if I WAS put onto TRT and the moron had to look on google to see what was available, because he confessed that he had never had to prescribe it before. To add insult to injury, he said that I could probably benefit from an injection of Sustanon every 2 months, and he would do me a favour and give me a prescription, but I would have to pay for it out of my own pocket if he did.

I told him to shove it where the sun don't shine. All I use the doctor for now is to take blood and send it off to the lab and I make damn sure that I get the tests that I want and none of his "Standard" bullshit.

As I said before, it's YOUR life and if you want to stay sick, be my guest. If you want to believe the medical profession want you to be healthy, dream on. I fixed myself in spite of doctors and Endos. I'm really enjoying life and trying to have sex as often as possible. I feel great, no one forces you to do so, enjoy wallowing in self pity. Not my problem.

So just because you had terrible doctors it means that Total Testosterone is worthless? Can you help me understand that logic?

By the way, I know we give doctors a lot of grief on this forum, but many of them are very good and want to help their patients.
