Need help interpreting blood test results (full panel)

Yo,

I recently got my full panel test results back. Unfortunately I don't have a doc I really trust with this stuff and I have only recently started reading up myself.

Can you guys lend a hand interpreting this? I got these tests because of gyno and libido issues (after a propecia crash two years ago).

Testo 528 ng/dl (300-800)
Free T 15.4 ng/dl (9-30)
FHBG 19.2 nmol/l (17.1-77.6)
Estro 37.15 pg/ml (7.63-42.6)
Vit. D 21.35 ng/ml (>30)
FSH 5.81 mIU/ml (1.5-12.4)
Prolactin 14.02 ng/ml (4.04-15.2)
LH 7.63 mIU/ml (1.7-8.6)
IGF-1 157 ng/ml (115-307)

Thyroid panel:

FT4 1.37 ng/dL (0.89-1.76)
FT3 3.48 pg/mL (2.30-4.20)
Total T3 1.16 ng/mL (0.58-1.59)
Total T4 5.70 ug/dL (4.5-10.90)
TSH 2.380 uIU/mL (0.55-4.78)

Estrogen test was not sensitive (wasn't available).

Thoughts?

Hi Scarface. It helps if you include reference ranges for the results. But going off memory I am not seeing anything that looks bad. TT and E2 look good. TSH is OK. Prolactin seems OK if memory serves.

Your Vitamin D is low though. You should supplement and get more sun exposure.

How old are you?
 
Thanks man... I'm 34... I just edited the post and added the reference ranges I was given by the lab.

I wasn't sure how trustworthy those are... for example, for testosterone I've seen 300 to 800 but I hear in the 1950s before the onslaught of xenoestrogen it was 800 to 1200 and that's a range I'd be more comfortable in.

You are right that Vitamin D is the only one that is outside the normal ranges. I started supplementing 10k/day and trying to get more sun too.

Some other numbers are in the range but towards the high or the low end. Something's definitely wrong as my libido has suffered significantly and I got gynecomastia (not horrible but quite annoying... looks like well developed pecs but it's soft).

I was also wondering (not sure if anyone knows this) if it makes sense to get gyno surgery now... or if I should get my hormones in a better range first so it won't grow back after the surgery.

What you described sounds like pseudo-gynecomastia. Not the real gynecomastia. In other words, it sounds like fat accumulation. Have you been examined and diagnosed by a doctor for gyno?

The average TT for someone your age is about 600ng/dl. So I don't think hormones are your problem. I wish it was as it would be easy to fix.

What is your height, weight and body fat %? Do you have sleep apnea or other sleep issues? What is your diet like? How much exercise do you get?

Are you still taking propecia? I ask because your prolactin is on the high end of the normal range. Highish prolactin might be the issue here.

Yeah... but I'm not sure how competent she was. She was an endocrynologist in Thailand... she said it was probably gyno because it's so soft. Also the nipple looks a bit pointy if that makes sense... And when I lean over (for example over the sink to brush my teeth) the moobs are really hanging. Shouldn't really be the case cause my body fat is only around 15%?

Thanks... yeah I think it's still not completely understood what propecia fucks up exactly, and I think maybe it does more damage in the brain than it does to hormone levels. I will say that I used to be above average T, and sex was on my mind a lot. Now I don't give a shit (I quit propecia 2 years ago and all that happened at that time).

I'm 6'1 and weigh about 208 pounds. Sleep apnea is probably an issue, but not a recent one... have been a snorer since I was a teenager. I was getting exercise almost daily when these problems started (resistance training and HiiT)... I eat healthy most the time now because I have a personal chef so it's easier.

Interesting... How would I best fix that? I quit propecia almost 2 years ago.

Values towards the edges of the ranges are:

Estrogen (in range but high)
FHBG (in range but low)
Vit. D (below range)
Prolactin (in range but high)
LH (in range but high)
LGF-1 (in range but low)


#IMT Staff: What's AI? My free testosterone is actually pretty decent on a percentage basis I think? 2.9% of total is free.

Thanks man... well that would be good news if that's true. I don't think I had as much moobs before as I do now with 15% bodyfat. Still, something worth trying before I go under the knife. I need to get shredded again anyway... been a while since I could see my abs.

It's not uncommon for bodyfat patterns to change. Dieting down or bulking up to 15% on two different occasions can have bodyfat distributed slightly differently. Especially common with hormonal inbalances. I think you just need to diet down until your happy.
