Red spots on arms/shoulders/back - not acne

If they are puffy they are than likely an allergic reaction to something. I get large puffy welt like red spots if I take a Penicillin, or Sulfa antibiotic. Those, don't look puffy though.
 
I'm not familiar with Cetirizine. Just read about it, and it says it can impair your thinking and reaction time. Not sure if they're just talking about drowsiness as is common with antihistamines. It dawned on me that I didn't think about flushing. Are you taking anything that might make you flush? You said you experience it more after you sweat, which means you're hot. This could just be a heat rash, or flushing. I would assume if this hasn't happened to you your whole life then you're taking something that is causing you to get a heat rash, or flush when hot.
 
I have those things when I get out of the shower. I don't really get too bothered because they dissipate after about 30 minutes.

If you look at tury's response that's exactly what I'm thinking of, a heat response. What could make you flush/heat rash could be any number of things that you personally have that reaction to. Some random examples would be Niacin, Melanotan II, Beta-Alanine in high doses. Think of EVERY thing you take, what has been added recently around the time this started. You may want to remove that and confirm that's causing it, and it really isn't a harmful side effect.

This also could just be, because temperatures are starting to typically warm up, and you are hotter than usual.

Yes, this is exactly correct. I notice that if I use hot water it happens more often, or if I start sweating while training outside and it is humid. It happens more often when I am on cycle and my guess is that it is because my body temperature is higher than normal while on. I don't think that op has much too worry about and if it was an allergic reaction Benadryl would have knocked it out as it is one of the most powerful antihistamines in existence (I know it is otc but ask any doc and they will vouch for its effectiveness).
 
Believe it or not I had the same reaction to Oxyclean laundry detergent! Head to toe just the same.
I went to doc got a shot in my ass and was gone I 3 days. My wife is nurse and she concurs. Make a list of things that could cause skin irritation and narrow it down, especially anything new or different that you have used or injected! Simplify and you will identify.
