Test Results - I think my E2 might be low

Hi guys, long time lurker and reader. I try and research as much as possible with out clogging up the forums with the same old questions. I have been on TRT for about a year now and just trying to get everything dialed in. I have a DR that allows me to pin at home so that helps. He only prescribes the Test the rest I have added. I was trying to use a mail order clinic but it didnt work out and was cost prohibitive.

Current Protocol:

.25ml test cyp (200mg/ml) on Mon/thur
.25mg of Anastrozole on Mon only
400iu of HCG on Mon/Thur

Current test done by labcorp- I think I ordered the wrong e2 sensitive test as the range is wrong. The test was taken on Wed about 10am 2 days after my last pin.

Any thoughts on the below test results, mainly the e2 as I have feeling its low and I was thinking about dropping the ai.

Testosterone, Total, LC/MS 698.1 348.0-1197.0 ng/dL 02
Free Testosterone(Direct) >50.0 HIGH 8.7-25.1 pg/mL 02
**Please note reference interval change**

LH
0.7 LOW 1.7-8.6 mIU/mL 01

FSH
0.6 LOW 1.5-12.4 mIU/mL 01

Estradiol 18.8
7.6-42.6 pg/mL 01
Roche ECLIA methodology

Estradiol, Sensitive 13.7
8.0-35.0 pg/mL 02

Sex Horm Binding Glob, Serum 17.9
16.5-55.9 nmol/L 01

Numbers aren't bad. Many guys on 100 mg a week don't need an AI. Perhaps drop the AI and get some new blood work after a couple weeks.

Thats kinda what I was thinking, but was just worried about keeping the e2 in check. I know the HCG raises E2 and the AI helps lower the SBHG from what I was reading on here.

Also I did have high E2 numbers when I started all the trt which was the reason why I was put on the ai by the clinic since my doc was not willing to listen to me. I was in the high 60's on the standard e2 test. Just worried about it climbing back up there. Wish I could but that little pill into smaller portions lol.

I appreciate the quick reply by the way.

If your estradiol is at 13.7 that is low. At 50mg of Test Cyp 2x per week, you probably don't need an AI. I would try dropping it all together and I think you'll find an AI is not needed. When I started TRT I was doing 100mg 1x per week of Test Cyp. At trough my e2 was 39. At 50mg 2x per week your peak and valley should actually be lower, because you aren't taking as much Test all at once.

Your E2 does look too low. Regular Assay puts you at 19 and Sensitive Assay puts you at 14 (although I agree that reference range looks weird). Most guys like having the Sensitive Assay fall between 20-30pg/ml. I agree that you probably do not need an AI on your current protocol. If you do see E2 creep up a little too high without an AI try using DIM and Zinc. They are not as strong but will help lower your E2. But I don't think you will even need that based on the numbers above.

Thanks for all the replys, i will drop the ai and retest here in 2-3weeks and see what it looks like. I will also double check the e2 sensitive test that i ordered.

Update-

Ordered up the Hormone panel for males, will try and get in tomorrow morning to get the tests done. I ordered this test as it had TT/hemocrit/E-sensitive. Since I stopped taking the AI I have noticed more retention in water, not bloated but definitely can tell the difference.

Labcorp changed their sensitive test. Better test now, and the range changed.

On sensitive you want to be around 28-30, but yes how you feel does come first.

When you elevate T it should do the same to E2, in order to keep the ratio proportionate.

OK, heres a updated lab with me taking no AI. The blood was drawn on the day of my next shot so 3.5days after the last where the other test was done 2 days after. Things I noticed is that my Free T has dropped a bunch from the last test. I assume this is due to there being more estrogen in my system and perhaps my shbg has gone up as well? Should have tested for that again as well.

You should donate blood as soon as you can. Hematocrit is getting high.

Estradiol looks great. TT could be much higher though... Personally I'd like to see it 800-1000ng/dl... how do you feel?

Sorry for not getting back to you sooner, been crazy busy the last week. I know I need to get the hemo down, its been high like that since starting TRT, I just need to find a place to do it as I live in a small town and cant just drive down and get it done.

When I was taking the AI, I felt ok but had the typical ED symptoms which prompted me to go and get checked and start this topic. I feel ok now but still feel about the same which I assume is because now my T has dropped. I was really hopeing that my Free T would stay high after dropping the AI...but it did not.

Are you a relatively lean guy?
I do 50 mg E3D. ...
try that instead if mon/Thursday shots...
comes to 500 mg a month exactly....
your e2 is nice.
you need to test FREE T4 and FREE T3 as well for thyroid.
dont get too comfy...
the longer you are on without an a.i. your e2 may climb.
400 iu HCG x 2 is plenty to raise e2 in addition to TRT...
keep an eye on it.

Appreciate the reply, i am not lean. Dont know body percentage for fat but i got a gut. Probably about 30lbs over weight...but have lost 20lbs in the last 6mo so its a work in progress and i have always been a big guy. Lowest weight i was ever at was about 220ish when i was in highschool and you could see my rib cage...if I can get to 230 i would be happy.

T4- 7.7 (4.5-12 ug/dl)
T3 uptake- 36 (24-39%)
Free thyroxine index- 2.8 (1.2-4.9)

I have also considered dropping the hcg due to the increase in estrogen...but i feel that it really does help my over all well being. Kind of in a pickle right now with all of this. Might need to just find a really low dose of ai or something less aggressive as it appears i am sensitive to it.

But your E2 is low now -- not high.
