Testosterone Enanthate - help?

This will seem like a stupid question to many, but there is a testosterone container that is unlike what I see in all the guidance videos online:

I want to check, which end is the needle actually supposed to go into?
Also, like with an ordinary vial, would you need to turn it upside down and/or inject the same amount of air as the testosterone you are extracting?
