31, low test symptoms, just got labs

just got labs in the other day. unfortunately they only tested total and free testosterone. they checked my thyroid and that came back normal. my total test was 400 ( scale was 250 - 1100) and my free test was 60. going back tomorrow cause doc wants to discuss options with me. any input would be appriciated

What do you want to hear bro? That's low for 31, but symptoms are more important. Are you lethargic, do you have low libido, lack of concentration etc...?
 
I would take it as a positive sign that he wants you to come back in to discuss options, I guess. I went through the same thing with my GP initially as my total test was low at 322, but over the bottom of the range, which was 241. I am 43 and he felt 322 was low enough to cause symptoms of Low T. I would think the same could be said for you, especially at your age.

Assuming he is willing to treat you, the next question is whether or not he understands how to treat low t properly. My GP did not and I now work with Maximus. Problem solved.
 
You want to pursue injections, prefereably with Human Chorionic Gonadotropin (HCG) and an Aromatase inhibitor (AI). If your doc offers the gel I would suggest you point out some of the many drawbacks, such as: long absorption times, rapid aromatization times once absorbed into the body and low net increase in test (most people see an increase of about 50pts - esta no bueno).

If your Estro came back within normal ranges he may hold off on the Aromatase Inhibitor (Nolvadex, Arimadex or sim.) scripp.

Other things for you to monitor on future bloodwork will be your Luteinizing Hormone (LH), Follicle Stimulating Hormone (FSH), SHBG and Estro. TRT/HRT will do you no good if it just converts (Aromatizes) to estrogen in your body, hence it's a good idea to monitor that. LH & FSH kind of act as the traffic cops for stimulating your testes to produce Testosterone. If their low, nothing much will get out and Human Chorionic Gonadotropin (HCG) can help to increase these numbers. Excess Test can and will aromatize into Estrogen so monitoring your overall Estro is a good idea (High Estro can be as bad or worse than Low T).

I will let others speak more to the SHBG values as it's late and I will certainly leave something out if I attempt to elucidate on that more than it being another Sex Hormone.

Thanks for all the input. The doctor is only giving me the option of androgel by itself at one pump a day and then in six weeks we will draw blood and change protocols if necessary.
 
Should I give this a try or find another doctor. My current one wont give me the option of injections until I do 6 weeks on androgel

That aside, as others have pointed out already, testosterone by itself isn't a suitable long term solution.

What I can tell you, from my own experience with gel based products (Testim) is that you may have the opportunity to determine how you respond to treatment before making longer term decision about real Hormone Replacement Therapy (HRT). I was able to get my level high enough on two tubes of testim per day to feel the benefits of TRT. Unfortunately, despite the fact that I felt great, my doctor thought my levels were TOO high and wanted to reduce my dosage. It was at that time I began educating myself about treatment and ended up with Maximus.

At some point, your T will aromatise and convert to estrogen then you are right back where you started. I used Testim just long enough to feel the benefits of increased testosterone, but not the side effects of increased estro. I was on two tubes a day for approx. 3 months before Maximus. I can't say you will have the same experience as everyone responds differently.

Or you could skip the whole gel process and go straight to Maximus, which could save you a TON of time. I started Testim based therapy in January of 2011 and didn't make it to Maximus until August of 2011. If I could do it over, I would have gone straight to Chip.

Yes 800 is the target. Is there something available otc I can take to lower estrogen while using androgel? still havent decided if im gonna even use this.
 
out of curiosity is one pump of this able to cause hpta suppression? one pump contains 20.25 mg of testosterone where as four pumps contain 81 mg. this is the newer version i was prescribed androgel 1.62%

But for sure, you need more labs: at least FSH/LH and estradiol; SHBG wouldn't hurt either.

Your 400 level has promise, particularly at your age. Might be worth exploring HCG+AI or clomid before you jump into the really serious stuff - especially when you consider the therapy will be for next 50 years or so.

I'm sure Chip could help you with the choice if your Dr. seems over his head.

Yes. However how suppressive it is on your HPTA axis is an individual thing.
