I am a physiotherapist in the UK and have been manipulating shoulders with this condition when appropriate with good effect. My experience with this condition is that the vast majority of patients with this condition have postural and muscle imbalances. Common postural imbalances are protracted, rounded shoulders with an anteriorly, medially rotated humeral head. often increased thoracic kyphosis. Abducted anteriorly tilted scapulae. Common muscle imbalances are weak rotator cuff especially the posterior inferior cuff. weak lengthened rhomboids + serratus anterior. Overactive tight pectorals (mainly pec minor) upper-traps and levator scapulae.

Whats does this all mean in English.... Identify which of these conditions you have and appropriately treat. Virtually every patient I have seen has weak external rotators (infraspinatus and teres minor) it is essential that you work hard on training these muscles, to complicated to say why. Hope this helps a little hard to give advice on net as there is so much variance in these patients.

My shoulders were not weak.. In fact probably my best body part.. But the doc said I prob have a tear which was a long time coming.. Years of gymnastics and years of weight training.. Overuse of my shoulders.. My right shoulder went out on my about 4 years ago; plus I have a winged scap on the right side.. Now it is my left rotator..

Cortisone shots were something I was thinking about..

I also use electric stim once a day.

I just want this healed up so I can get training again..

Thanks for the feedback

Damn Sup..
That was some deep stuff right there..Sounds like he knows his shi- though..
Nothing bugs me more than not being able to train..It s my 60-90 minutes a day thats is ALL mine.
I don t push..but, cortisone helped my left elbow with one shot..1986 ! I took 2 shots in 1992 for my left rotator..still no pain...fyi.
I followed both these with a slow graduated return to lifting....no regrets for gettin em.
I need some in my left knee now ! ( The hits keep coming..but I m 45.)
I wish you and ur shoulders well..

You can have powerful shoulders but still have muscle imbalances. Inferior angle winging of the scapulae is almost always due to weak serratus anterior + tightness in pec minor.

Cortisone injections take away the inflamation the main cause of pain. In otherwords it treats the symptoms but not the pathology. Sometimes can help but really need to treat the pathology otherwise the benefit will be temp. I would seriously consider seeing a physio who specializes in shoulder pathologies.

If you have a rotator cuff tear it can be easily and relatively cheaply diagnosed with an ultrasound scan.

i had the same thing, nothing helped, i couldnt press any weight over head, did the exercises, they wanted to do the impingement surgery but grinding bone off my shoulder joint just dosent seem good. I had been off stuff for a few years prior and got some deca and some test and no joke it went away. I guess the deca may have done something. Im not a Dr nor advising that Deca is the cure all for impingement syndrome but it cured mine. 5 yrs later and my shoulders are my best BP.
But i didnt have a tear, so make sure yours is not torn, get MRI and go from there. Hope you feel better, i know i was in alot of pain.

I've had the same thing. It's a nightmare. Rehab, electric stim, massages & most importantly time off will get it better. Mine took 2-3 months to stop hurting and another 2-3 months to not hurt when lifting. Don't do chest exercises & def don't train your shoulders at all. Also, be sure not to sleep on them as the pressure & weight from sleeping on them will not allow it to heal. For some side sleeping is a big cause of the problem. Glad to say my shoulders issues went away but i still only do shoulders every other week to keep it that way

hope u feel better soon. Hang in there

OMG the side sleeping!! I can't lay on my left side.. well apparently I sleep in a "flamingo" position.. one leg up and one arm up above my head lol.. I can NOT get my left arm up without pain so now I only sleep on my right side or stomach.. it's a bitch..
My pain is not as bad as it was but I also have not trained in a month.. I still have absolutely NO strength when lifting overhead or out from my side which is pissing me off big time! I tried picking up my handbag from the seat and could barely lift it.. my bad ass 10lb Louis Vuitton ;)

I had some massage work done as per a suggestion of a chiropractor friend.. he thought that I probably also have some scar tissue built up which could be also causing pain and if I do have scar tissue and it gets broken up I would heal better.. I did feel relief the few days after the massage so I will try that again..

Thank you so much for all your input and for clarifying what you meant in regards to the tightened pec muscles etc.. My only confusion is that I hardly ever train chest.. I train heavy back and used to train heavy shoulders but not much chest.. some incline presses here and there but never my focus.. But I understand what you are saying.. So I would assume then that massaging my chest muscles and loosening them up would help my shoulders a bit..
Also, I am going to take your advice and see a specialist.. When you say a shoulder pathologist, do you mean an orthopedic? I am not sure who to contact.. I have used a sports medicine and chiropractor in the past. Thanks again for all your help with this.

My shoulders were my best body part too. I had to get the surgery to fix mine. Some how the rotator cuff tore when I was trying to get into the next level of the big bench club at my gym and it caused some bull shit in my shoulder to get degenerative joint disease. They had to go in and shave part of the bone off. The surgery was called an acromioplasty. It was really hard to recover from, but hey 3 years later and im finally as good as new.....
