A little background, for the last 4 years ive lost all sexual intrest, I feel like ive been castrated and cannot get horny or in the mood at all, even when I meet an extremely attractive girl and have her clinging around my neck in a club, or end up back at her place... there is zero male sexual hormone flucuation & activity along with zero neurotransmitter activity...Zero sex drive, premature ejaculation, and lack of morning wood (3/7 days ill get morning wood) Id rather watch tv or read a book than have sex with a woman, and its starting to cause issues for partners, so much so ive ended up breaking with girlfriends because its not fair on them not being satisfied (intercourse)

My life has become aimless and non goal orientated, I feel as if im merely exsisting and no more, I have spouts of anxiety through the week and have lost all assertivness, I feel less of a man daily and have started to become emotional at little things and starting to feel feminine, its really a mindfhk. At work id rather sit next to and feel more comfortable with the women than sit with the guys. Ive gained a high% bodyfat although im quite light, and it is carried on my hips, waist and chest. 170lbs, 6ft, 22-25% BF

I do not smoke, and I do not drink.

Over the past 6 months ive started to develop short term memory issues, forgetting where ive placd a set of keys 5 minutes previously, and spending half an hour looking for them, and developed brain fog, where I can be in the middle of a conversation and its just a cloud of confusion, I cannot remember what I am saying, and cannot spit the words out ? Night sweats have also been on and off for the last 2 months.

I went to my doctor 12 months ago and had bloods done, they came back with a basic testosterone value; 12.5nmol. I was told this was normal and sent away.

I changed my lifestyle for 6 month period, ate healthy, started cardio 5 times a week and lifting x3 a week. I supplemented with omega 3, vit D and lost 36lbs. After all of this change I still felt terrible and there was no change. I really struggled with weight lifting and with cardio, even after 6 months of consistancey.

I returned to the doctor, only this time a different person after 6 months of 'lifestyle change' and explained everything to him, he agreed that a male of 21 should not have lost intrested in sex etc, he ordered another basic testosterone test which can back at 13.7nmol My testosterone had increased by 1.1nmol. He agreed to send me to a specialist.

September 2011 - 12.5mnol
August 2012 - 13.5nmol
January 2013 - 10.7nmol


I saw the endo in January and had full blood work taken, the results are below (note these were taken at 09.20)

He said that the ' testosterone is a bit low but normal' and that the 'estrogen is a bit high'. He also said hes confused as to why the lh and fsh is a pretty good value but the other values ( T & E) are out... I really dont hold much hope. he says that the answer isnt being put on testosterone replacement therapy (TRT), I really dont hold much hope...

Anyway I got bloods taken again yesterday and return to the endo in 3 weeks to discuss them again..... Any opinions on my readings ? any ideas on what would sort me out ?

You do not need testosterone, you need to get your estrogen under control, this will increase your TT.

With an aromatase inhibitor. Your body is converting a good chunk of your test into estrogen. An Aromatase inhibitor (AI) will stop this conversion, allowing your body to build up its testosterone levels.

I am curious how MUCH you gain from an Aromatase inhibitor (AI) though. (I suck at conversions, but isn't that like 305.73ng/dL TT and 56.11pg/dL E2?)
 
Do you have any previous cycle history because this is not normal. I was thinking that it was a thyroid problem but your tsh is fine. I agree with everyone else you need to get that e2 under control.

What kind of dosage suggestions do you guys suggest for him? Adex or arom? Talk to your dr about using an Aromatase inhibitor (AI) see what he says about the estradiol. Good luck.
 
I have never touched any controlled drugs or steriods, this has simply happend to me, or developed this way through puberty etc.

Also I am getting tested for karyotype XXY
 
Just curious. Did you go through puberty normally?

Do you have signs of gynecomastia?



Was extremely obese during preteen through untill late teens, suppose that coul have effected puberty. Doctor can feel non fatty tissue growth under nipples.

I have a feeling that may be what caused your high E2 levels as this wouldn't be the first time I've seen strange blood tests due to pubertal hormone imbalances. You're kind of in a tough place as a lot of doctors don't even know how vital it is to keep that aromatase conversion in check. If your doc discounts your estrogen being a cause for issues, it is probably time to start shopping for a new doctor.

If you're still overweight, dropping your bodyfat will likely help as well, but with your hormone levels being as they are - it's not going to be an easy journey. I truly wish you luck as you may have some tough times ahead.

I managed to loose all of the weight, from September 2011 - April 2012 I went from 220lbs @ 30% BF to 160lbs @ 18-20% BF

As you can imagine 160lbs at 20% bf, 6ft tall is completely mad and my body composition is really screwed, at 160lbs I still had massive stores of fat on my stomach, sides and back and had pretty much little to no muscle mass, this was pretty much my plateau where I didnt have any lean muscle mass to help burn the remaining fat, and I didnt have any Test to put on muscle, so a very vicious cycle.

Yes my testicles are the 2nd biggest on the string with different size testicle molds on it
 
I had results back from my latest bloods,

Testosterone = 9.6nmol (8.6-28.8)
SHGB = 12nmol (18-45)
Estrogen 222pmol/L (0-150)

Everything else was 'within range'

In the period over the last 14 months ive been having blood tests in an 'average' of 3.5 month intervals. My Testosterone level has dropped 1.1nmol every blood test interval again on 'average'

I am getting no treatment or therapy, they say my hormone levels are fine.

I have been given a script for viagra, but errections are not my problem? im confused
 
I thought I would post an update for everyone - I'm still stuck between a rock and a hard place...

I was put on a TRT trial for 7 months, it was a rough experience.

I actually feel better 'off' TRT than 'on' , even suffering the same symptoms, it was a hellish roller-coaster ride and I felt really unstable at times and it came with some sides I didn't enjoy.

- Little to no ejaculate volume (only enough to cover a pinky fingernail
- orgasm pleasure massively reduced

I went through this fun ride every injection cycle....

The first week after injection my symptoms were relieved, but I felt extremely depersonalized, hyper sensitive (hearing,sight) and wired.. sometimes I felt a little manic..

The second week after injection I felt amazingly good, mood, energy... sleep, sex. I felt normal

The third week after injection all of the low t symptoms returned.

They had me on 1ml of sustanon250 injected once every 3 weeks... with no AI or SERMS (UK - NHS)

A consultant endocrinologist took over my case from a jnr registrar when I told them I wasn't happy with the direction of treatment, and my symptoms were not improving. He took me off TRT so he could get a baseline reading himself and carry out investigations.

MRI on pituitary - OK
MRI on abdomen and pelvic area - OK
Cortisol suppression test - OK
Testicle ultrasound - OK
Androgen receptor sequence test - TBC
Genetic XXY test - Negative
Bone density test - TBC
Sperm count - 0 (3 weeks into TRT) needs re-test.

Does anyone have any opinions ? my symptoms are the following ;

Poor mood ; not happy, not sad. just tolerating. (not in a depressive state)
No enjoyment for hobbies or interests - no excitement, no laughter
No assertiveness, slight anxiety
Don't want to socialise
Very irritable with people
Poor sleep - extremely groggy every morning (doesn't lift by lunch time)
Poor short term memory , poor concentration, headspace feels cloudy (in a vegetated state)
general achiness (neck, shoulders, upper arms)
Lack of energy
ZERO LIBIDO (not interested in sex, never horny) Non existent sex drive, poor ejaculation (can get erections)

I've been told on another site that I have psychosocial issues and possibly depression and that my sex hormone values are fine?

Looking for opinions and views of others... I await my next appointment with the consultant in a week, to see where we can go from here.

I think folks tried to tell you before but you've got to get your E2 levels under control. Also, your protocol while you were on TRT is a big part of the reason you aren't feeling well. Shots every 3 weeks will cause a roller coaster with your hormones and will cause your E2 to increase even further. If your doctors won't do it, do it yourself. Get an AI.

Also, have you ever had a sleep study done?
 
Never had a sleep study done.

You need to understand that I was initially wanting to play by the books with my treatment/investigation, NHS don't provide oestrogen control, because high oestrogen is not a problem apparently.... so why do they test for it and have ranges ?

I have been offered Gels : tostran, Testim or testogel. Or Nebido 1000mg/3ml E12W.

Nebido seems to be a good option for many. See bigben's log where he highlights his experience with it. Although without controlling estrogen you are not going to fully experience the benefits to your health and well being.

Agreed, high E2 can cause all sorts of side effects, controlling your E2 is just as important as increasing your TT!
 
you had e2 problems before trt. Then when you started trt ya you felt good for a couple weeks but then I'm sure your e2 shot up again and made you feel awful again. You can pump tons of test in your body..if your e2 is high you will still feel like shit. Your body for some reason loves making e2. You honestly need to study up on the effects of e2 in males and provide your doctor with scientific documents showing what it does in men and why this is affecting you. Doctors only practice medicine..it is a constant study for them so you just need to provide the info so your doc can learn..if you have a good doc then they will gladly learn and apply.
