Everything else looks fine from the results, however the TSH is getting higher and T4 is still quite low...

Any opinions on what to suggest?

can you give some background info? age, current meds?

Age - 22
Issue - Head trauma at 15, pituitary liaison possibly causing low T but they never quite figured out the root cause but most likely, they were confused as to why its outputting TSH/GH fine but not T...
TRT - 125mg Test en every 7 days, or 250mg every 14 days. (4 months in)

Meds - No others

Felt a big improvement in mood/general welfare + labido since being on TRT, gained quite a bit of water 'fat' weight around my midsection and thought it may be high oestrogen, but now I'm thinking it could be my sluggish thyroid?

Since being tested for over 6 months my TSH has read 2.7-4.7 consistently, with T4 in the same range but docs keep shrugging it off as not an issue...

Thanks

Your Thyroid hormones aren't optimal. FT4 looks crappy.
TSH no good.
You didn't get FT3 done.

I had FT3 done blood test before and it was also crappy, 3.13 (2.6-5.7)...

Doc said he ddidnt want to be adding another long term med at this point given my age (22) but i do wonder if its masking most of the T improvements somewhat...

I know i don't have autoimmune disease from a previous test, so I'm tempted to try iodine replenishment for 2 weeks and see if that makes an improvement, i don't have any iodine in my diet currently from salt or kelp etc... so it could well be the primary cause

Are you dieting to lose weight? What is your height and weight? Any significant changes to your weight in the last year?

Im currently dieting, but on that blood test i had been in a surplus for over 5-6 months.

My weight doesn't really fluctuate much at all, although I'm very strict with my macros/weight changes and adjust accordingly. I have a big appetite and can gain weight easily.

Im dieting to lean up, not to loose weight although weight will come with that obviously.

Height 5.8
Weight 70kg

pictures attached, left is me at 14 (bit fat i know haha) and the other two were last summer which is what I'm trying to cut down to again, it did take a lot of effort diet wise and smart cardio/lifting...
If we say I'm 10% in those pics, id say I'm currently 13%(fluffy abs)

That was pre TRT and probs 4kg lighter

It's crap. I had 3.4 pre TRT.
Within a month of TRT it went to 5.7 ....

Get on some LO SALT iodized salt if sodium is a problem for yoU. Dulse Flakes have a s"""t load of iodine.

Wouldn't hCG alone stimulate DHEA production? Or would you need to supplement more than natural output?

yes may need to supplement more. or even pregnenolone. Some don't like preg though cause it can cause water retention. It should stimulate, but may not have adequate raw material.
