I need help with my blood test results

I'm not sure if I'm posting this in the correct section. I guess someone will move it to the correct one if not.

I'm 44 years old. 6ft 5in tall and I weigh around 105 kgs. Some years ago, I was a power lifter, but a very bad accident left me in a very bad condition for a long time.

Early this year, I started at the gym again. I'm not progressing as I should. I feel tired most of the time, I seem to be getting a "spare tyre" and I'm not getting much in the way of muscle.

I asked my doctor for a full blood test and I got it back from him yesterday. He says there is nothing wrong with me apart from my Cholesterol, but trying to compare these values with anything on the web or with other people is impossible because all the units are different.

Here is what I got back apart from the haematology :

Kidney Function
Urea - 38 mg/dl
Creatine - 1.07 mg/dL
Uric Acid - 4.2 mg/dL

Liver Functions
GOT - 17 U/L
GPT - 15 U/L
Gamma-GT - 38 U/L
LDR - 364 U/L

Lipids
Total Cholesterol - 241 mg/dL
HDL-Cholesterol - 45 mg/dL
Coronary Risk Factor - 5.4
TriGlycerides - 167 mg/dL
LDL-Cholesterol - 180 mg/dL
Ratio HDL:LDL - 4.0

Ions & Electrolytes
Sodium - 141 nmol/l
Potassium - 4.6 nmol/l
Chloride - 106 nmol/l
Bicarbonate - 27 nmol/l
ANION-GAP - 13 mEq/l

Inflammation
CRP Dosage - <3 mg/l
CRP Dosage - <0.3 mg/dl

Thyroid
TSH - 0.526 µg/ml

Hormones
LH - 4.4 U/L
FSH - 5.7 U/L
17-beta-estradiol - 15 ng/l
DHEA Sulphate - 121 µg/100
Total Testosterone - 400 ng/dl
SHBG - 0.99 µg/100
Free Testosterone Index - 4.04 Index

Tumor Markers
Prostate Specific Antigen - 1.37 µg/l

I'm not a doctor, so I have no idea what all of this means, only that I don't feel good. My wife and I had lots of problems and she left me, but my sex life is non-existent and to be honest, I don't have a lot of interest.

One of my friends says that my testosterone is low, but he has no idea what the Free Testosterone Index is all about. He says my thyroid isn't that good either. I have no idea.

I found this site on the web. I hope that someone here can help me out!

Can you add the reference ranges to your post? Every lab is different.

By the looks of it, your testosterone is a bit on the lowish side for your age, but not hypogonadal. You are at 400ng/dl. Average for your age is in the 500's to 600's.

Thyroid looks good.

Post the reference ranges and we can discern more.

How is your diet? How much sleep are you getting? Could you have untreated sleep apnea? Any testicular trauma or past cycles? You LH and FSH are a bit on the highish side it looks like which could indicate a problem with your testicles.

I didn't get any. No-one seems to know what Free a testosterone Index is with no units. I can't believe that my Free Testosterone is more than my total testosterone.

You have to have normal ranges for your labs. Can you post a picture of the results you for wi your personal info blacked out?

Free T is often expressed as a percentage. You didn't provide units though so I can't say what your 4.04 refers to.

What I got from my doctor I put in my post. He didn't give me the lab results, he must have just typed them out. The Free Testosterone was exactly as I showed it free testosterone index.

Looks like you need to get on the phone and get a copy of your results with normal ranges included. Should be an easy fix.

That FSH is up there....
i don't even know if clomid will help you much given that.
worth a try though.
what ranges are the DHEA and Estradiol?
They are not listed.
so you don't know your exact free t or bio avail.T level?

Free testosterone index is unitless and a garbage metric some labs use as they try to save money. It's essentially free test divided by SHBG and multiplied by 100. I'm a little rusty, but I'm pretty sure that the range for men is 30-70, which could explain a few things. I'd double check, but I'm on my phone in between classes Lol.

Everything else looks pretty good, and I agree that your lh/fsh is pretty high, indicating a potential issue further down the stream.

My .02c :)
