Looks like TRT may be in my future

After a few years of just dealing with the symptoms that I'm sure a lot of you guys are familiar with- body fat gain, irritability, inability to focus, ED, trouble staying motivated, always being tired- I finally asked my primary care physician for a test for my testosterone levels. He called me back and said it's low- I'm 32 and my total test was 220. He referred me to an endocrinologist, who I will be seeing in a few weeks. Is there anything specific I should know about before going in to see the endocrinologist? I've never had a problem with immunizations, needles don't bother me, although I have read that some docs like to use other meds besides Test Cyp like a patch or gel. I pretty much just want whatever will make me feel "right" again, so if anyone has any info that can keep me from having to trial and error my way through a bunch of different meds to find what works, if there even is a "best solution", I'd like to have the info to be prepared to talk to the doc when I go in.

not sure on bf%, looking at some pictures online, I'd guess between 25-30%

What I'm doing right now- my diet is pretty crappy. I eat a bowl of steel cut oats with one teaspoon of local honey once a day, and 3 times a day on my non working days I throw a couple of handfuls of pre made salad into a bowl and top it with 8-12 oz (a handful-ish) of boneless skinless chicken breast that I broil and dice every few days. At work, I get the cafeteria to make a wrap with one of those large tortillas and have them fill it with all the veggies on the line plus one boneless skinless chicken breast and a glug of extra virgin olive oil and red vinegar, then I open the wrap and eat it like a salad, discarding the tortilla. If I'm feeling particularly hungry, I get a box salad to go during lunch and save it for later (my work is 12 hour shifts at night, so no options if I don't stock up during lunch).

For exercise, I go to the gym across the street from my apartment and try to work through a routine around the gaggle of people in there. I go twice a week, but I know I need to go twice more per week, and I've been making excuses for not going. I don't do nearly enough cardio, only once a week jogging 2 miles around my neighborhood.

I have lost weight (40 lbs in the last 6 months), but I know it's not coming off as quickly as it could be if I were sticking to a rigid routine and keeping a better diet. I did eliminate drinks with calories completely, and the only things I drink that aren't water are those mio squirt things that you add to your water and once a work day I drink one of those zero calorie energy drinks

The issue I'm having with going to the gym is that I'm always so tired, and I know that in order to break the cycle, I need to just go and make myself do it
 
Your Endo will run more blood work to check thyroid, lipid, estradio, etc. to determine probable causes for your low count. From there they will begin a plan to bring it back up to normal levels. One may be a restart using clomid, others may be gel treatments. Also, actual TRT may also fall on what your insurance will cover. Mine would not cover the gel, but will cover test-c.
 
You should have your diet looked at.. Its likely that you're starving yourself..

Have you ever heard of Carb cycling?
 
as far as what my insurance will cover, for TRT, test enanthate and test cypionate are both ranked as lowest cost, Androderm, Android, testim, and Testred are ranked as preferred over highest tier, and Androgel, Axiron, Depo-testosterone, and Fortesta are ranked as highest cost, in the same category that they rank Viagra and Cialis.

As nice as it would be if test was a magic bullet that would remedy a bunch of my symptoms, I really want to do whatever will make me feel more like I felt 10 years ago. If that's pinning test, then ok. If I need some sort of thyroid medication and that takes care of it, then I'll be stoked for that. I'm just tired of feeling like this all the time
 
3J, no the first time I heard of carb cycling was when I clicked the link in your signature. I am tempted to put down the cash to get onto one of your diet plans. I know there's a connection (at least for me) between dropping hard earned cash on something and the motivation to see it through. When I paid a personal trainer, I sure as hell didn't miss any workouts

my diets help peoples overall health.. if the issue is truly from your nutrition.. ill help fix it
 
Just wondering if you have kids? The reason that I ask is because when my Endo suggested the gel I told her I had little ones and didn't want to risk transferring any medication to them and told her I prefer injections, she didn't even flinch.

Yeah, I've got 3. 2 of them are still small enough that they like being carried around
 
Also, some insurance companies will no longer cover Andrgel and Testim. My first month of Androgel was $500 due to lack of coverage. needless to say, I did not get it. This will probably get you on Test-C more easily.

Update- I really need to procrastinate less with my health. I got a big No from the doc, he said my T levels were at the low end of normal with a total test of 220 and a free of 9, the bioavailable was around 50, he said everything was at the low end of normal. I figured it was time for a second opinion, so I got one. By the time I made it to the lab for this one, I came back with a total T of 162 and 2 days later, the doc's office called me to tell me I have a prescription for Test Cypionate and an appointment to learn how to inject in a couple of weeks. Maybe now I can get my head on straight and go back to wanting to do...anything

I got a big No from the doc, he said my T levels were at the low end of normal with a total test of 220 and a free of 9, the bioavailable was around 50, he said everything was at the low end of normal. Good luck! Getting dialed in has been a process for me. Hopefully it will be easy/fast for you.
 
Picked up my prescriptions today. Looks like I'm starting at 200mg every 10 days. I tried to go to the pharmacy and get some extra syringes and needles so I could split that down to every 3-4 days on my own, but after a whole bunch of run-around (CVS apparently doesn't like to sell syringes) the pharmacy I went to didn't have any 18 gauge, just 25. I don't think I'm supposed to start injecting until after my appointment with the doc where I learn how to inject anyways, so I guess I have some time to get some syringes and needles...maybe I'll just order them online
 
Had my first dose today. Went to the doc's office and he gave me the rundown of prepping the syringe, vial, needle, and to get a sharps container.

He demonstrated how to draw with one of my 18 gauge needles and showed how to cap and twist off the needle and replace it with a new 25 gauge, both of which he prescribed me with the test cyp. He had me pinch an area on my upper right thigh with my left hand and had me hold the needle kind of like a dart in my right hand and tools me to go ahead and stick it in all the way.

I took breath and sighed, and he said most guys hesitate at first and it takes them a while to get the motivation to do it. I said something like "guess I have to do this if I want to fix my low t" and just kind of pushed it in like I was tacking a pin to a cork board to hold a card or something. I was surprised that I didn't feel anything at all when it went in, there was a little resistance at the skin surface and a little more about a quarter to a half inch in where I'm guessing the muscle started, but it wasn't just painless,

I couldn't tell that it was in at all without looking at it. It did go in crooked and he said to go ahead and push the plunger in with a good amount of force, because the oil is very viscous. I asked is the needle was in sub cutaneous fat and he laughed and said no there's only about a quarter inch of fat there even though it's pinched, it's definitely in the muscle. He didn't have me aspirate, and it took a while to push all the solution into my leg. It was pretty cold but it was only 1 ml so it didn't make a knot or anything.

Small drop of blood when the needle came back out and then he wrote down my regimen for me. 1 ml (200 mg) every 10 days with blood drawn the day before the 3rd dose. If my levels come back high, he'll extend me out to every 2 weeks same dosage, if they're still low or completely nonexistent from being shut down and burning through the 200 mg every 10 days, he'll dial it down to the same dosage but weekly instead of 10 days, and if they're good, which he expects, he'll leave me at 200 mg every 10 days, which he said is a little aggressive, but most of his patients end up having optimal levels at 200 mg every 10 days, so that's where he's starting me. No idea when I'm going to start feeling better, but I'm looking forward to it. I forgot to ask him, but I asked one of the nurses and she said maybe a couple of weeks to start feeling a little better.

One thing I don't like, and the doc didn't either was that the test is in 1ml vials. Good for keeping sterile I guess, but they're tiny, and look easy to drop. Next time I see him I'll ask about cutting the dosage in half and injecting twice as often
 
Glad to hear you're on the road to recovery! Congrats :)

If I was you I'd just start injecting twice per week straight away. The problem with less frequent injections is that it makes dialing in properly almost impossible. If you dial in your TRT based on trough readings (the blood drawn the day before your injection) on such an infrequent schedule, your peak test and estrodial levels will be way above range. Not good.

If you were to try dialing in on the peak readings, at your trough you would be basically hypogonadal again. Also not good. You really need to have stable levels throughout the week, which only comes from twice weekly injections. Dial in like that and your dialed in 24/7/365.

Also, no need to pinch on injection.

Something to consider anyhow :)

Problem is, the same pharmacy that I get my syringes, needles, and test from won't sell me any more syringes besides the ones I have a scrip for. None of the other pharmacies in town save for one will sell me any, and that one will sell me 25 gauge but doesn't carry 18 gauge
