Still feel crappy at 4 ½ months on TRT!! Is this normal?

210 lb 63 year old male on TRT, the Canadian way, with whatever pluses and minuses that involves
Quick history: 4.5 months ago before start of TRT, blood test for total T was 8.5 pg (8.4-28) or 245 ng/dl and free was 1.5 (2.1-8.5) which is about the average to a 100 year old man. I had severe low T problems. For the past few years, my mind was foggy, unable to focus, poor cognitive ability (Basically, I had gone from being young and smart to old and stupid), somewhat depressed, and all the other problems that goes with low T.

So 4.5 months ago, after blood test, Dr. started me on 100 mg daily cream or gel which is equivalent to 100mg of cypionate every two weeks with an estrogen blocker.

After 2 months on TRT I felt no improvement at all. I got the blood tests done on my own and the test results showed very, very, minor improvement (total was 11 and free was 2.0). Basically no change. Dr increased to 150 mg daily dose of cream which is equivalent of 150 mg of cyp every two weeks. And told me to be patient, as he said it was only two months.

After 4 months on TRT, I still felt very very slight improvement. Again I did a blood test on my own and there was no improvement (total 11 and free 2.0). He tells me that he is not surprised at test results as “blood serum numbers are not reliable on creams”. And “to be patient”. So I asked Dr to switch me to injections. At 4 months I start 75mg of cyp test self-injected weekly (that’s 150 mg for two weeks), plus estrogen blocker. So same dosage, but now injected.

After 3 three weeks (4th 75mg injection), I still only feel no difference. Is this normal??
Dr. is a nice guy, but do most doctors take this long to increase dosage, when dosage is not working?
Lastly, he told me dosage 200 – 400 per two week period is basically overdosing. I’m a little afraid that if 200 EOW does not fix me, I’ll be out of luck. Anyone hear of TRT dosage going over 200 EOW in good ole conservative Canada?

Did you get complete labs done? Everybody is different and in Canada I hear it's harder to get TRT so try to work with the doctor and explain you still have the low T symptoms hopefully he will increase your dosage. Did your labs show you needed an AI on that dosage of Testosterone or do you have high E symptoms?

In Canada I haven't met too many docs doing so...
increase dosage. Seems the few I seen want to sell you short on T as much as they can.
Had one Endo say 480 mg/dl is fine on HCG mono protocol.

I just had lab work from family doctor and took results to the anti-aging doctor who is now treating me. Only had free and total T and I believe estrial. Anti-aging Dr started me immediately on T as my symptoms were so bad. I'm overweight so I think I would have gotten the A1 anyways. I don't know the estrial number. Don't think I had high E symptoms. Is foggy brain a high E symptom, as I did have that? That's the only thing the treatment has fixed.

Next appointment is in three weeks. I assume Dr will increase dose a little or figure something out, as the bottles might as well be full of just water. And if 150 mg for two weeks is doing nothing for me, I have a hard time thinking that 200 mg will make such a huge difference.

Sorry to hear that! Canada seems to be rough for TRT and there seems to be a large demand for some good docs.

Brainfog can be either estradiol or low t. Sounds like your treatment is going forward, just try to keep your hopes up and as Apollon has experienced it is frustrating!
 
What are your Test levels on 75 mg/week?

75 mg/week is on the low side of the dose range. 100 mg/week is more normal. 200 mg/week is avove average.

My test levels on 150 daily mg compound gel (equivalent to 75 mg of test weekly) was total 11nm which is (317ng/dl) and free was 2nm which is (58ng/dl).
Before TRT I was 245 and 40 free.

I have not been tested on cyp test injections yet, as it has only been over 3 weeks. So unless injections are more potent, my numbers will still be 317 and 58. All tests done so far have been done by me. TRT Dr has not asked me to do any tests of any kind. Seems odd to me. And after 4.5 months, I'm no better and only on 75 mg injection. Yet I hear of others in USA starting at, at least 100 mg per week.
 
The Free T numbers I gave must be wrong, so the reply below to Virginian must be wrong: I used a online calculator to convert Canadian to USA units. My free is lower than normal range, but I do not know how to convert 2 nmol/L to ng/dl as I think the conversion number I received (58 ng/dl) seems to be really high.
The Total T was correct.

This seems like what i see so often in regards to the androgel. Results always seem to be very poor and not even close to the results people have on the injectable Test cyp. treatment. Thankfully Now adays though there are alot of online clinics that will monitor you through labs that you have done every so many months and will give you a decent dose that will actually make a difference. This is the exact reason i chose to go with that option. Alot of doctors are just not caught up with the latest information and science regarding Trt/Hrt.
