Waiting on corisol levels to start HRT, at the end of my rope.

Hi guys im a 28 yr old male and have been diagnosed with low T along with other problems.

Ive been to the endo last week and the first thing he said to me was "your bloods are all over the place". My first reading last year was 8.9 nmol/257ng/dl, 9nmoll being the end of the scale, I took a second one 2 weeks ago and its its now 5.6 nmol/162ng/dl !

Needless to say im feeling like crap. My B12 reading were off as well and my creatinine levels are higher than the highest levels so theres probably something wrong with my adrenals as well along with the fact ive been suffering from anxiety and panic attacks sine I was 19.

He said he would give me my prescription on that day but without knowing my cortisol levels he couldnt as if they were wrong or out of whack and I started TRT it would make me worse as my body wouldnt be able to handle it.

So I took bloods 2 days ago for cortisol along wit T again and some other hormones.

These were the other hormones the endo requested fasted at 9am..

Cortisol
Testosterone
FSN LU prolactin
TFT'S
UDE UFT'S FBC


I hope I wrote them correct, trying to read his handwriting as best possible.

Those should be back to me next wednesday and my appointment with the endo is on the 30th.

My question is,if my cortisol is wrong and he starts me on treatment for that, would it have to be fixed first before he gives me TRT or is it a case where you can start both at the same time?
 
Make sure you check LH and FSH so we van figure out what type of hypogonadism you have.

When you get your blood work results post them up with normal ranges and we will help you figure out what is going on.

Get Estradiol tested. Your levels are real low.

No im in Ireland. I know my levels are low man, I feel terrible :(

Well im just doing what the endo tells me at this stage and want to start TRT asap. If I ask him to start testing more things like Estradiol wont I just be delaying getting my treatment?

Thanks for the replies guys but back to my main question, if I have to start Cortisol replacement, can that be done at the same time as TRT or will it have to be fixed first before I can start TRT?

FSH and LH, yeah it was his handwriting, he has them in the list I mentioned in the op but I had them down as FSU and LU.

Ill let you guys know the results as soon as I get them, Wednesday at the earliest.

My guess is that most doctors would rather treat one thing at a time. Starting 2 treatments at the same time would make it a little more difficult to figure out which one is helping you, or hurting you if things don't go well.

Your age will also likely play into his treatment decisions. Since you're a young guy, he's probably going to spend a bit more time trying to identify the underlying cause of your abnormal bloodwork. In older males it's a lot easier to chalk low T up to andropause.
 
He did say to me that he would of gave me my prescription there and then, but he couldnt as he didnt know how my cortisol levels were and if it was wrong that the T would do me more harm than good, so he sounds pretty willing to get my on the T.

I've lived at 180 ng/dl briefly while coming off HRT for restart. It's the same as being castrated....
Get it fixed fast bro.

Apollon, seriously...., you should write a book. After all you have gone thru, you definitely have enough content to fill a book. Perhaps you could shake up the medical industry a bit.

WhatTRTmayCome. I can't imagine how bad it must be at 180ng/dl. Hang in there. It will be OK.

As long as you have low estradiol its not the worst it can get.
I had 38.4 pg/ml Estradiol with 180 TT. I felt anger, rage etc during that period i lived with those levels.

I can kinda understand that. If you were to have adrenal insufficiency, TRT could cause problems. AI is pretty rare though, and symptoms are usually very apparent. It's not something that's going to be diagnosed with a serum cortisol test though, but I guess that's a starting point. If your cortisol is super low, the next step would be an ACTH or cortrosyn stimulation test. Low cortisol is one thing, adrenal insufficiency is a different beast altogether.

And the Alopathic medical system does not recognize adrenal issues nor do they treat it let alone run such in depth tests to figure out what is going on there.

It was actually 162nmol last checked man, its gone down from 257nmol last year. I have all the symptoms and tbh I really am at the end of my rope and just trying to survive at this stage. Ive even been having suicidal thoughts regularly but im doing my best to block them out.

I havnt done an E test so its possible I have it. My symptoms include.

Anger
irritability
low focus
cognitive impairment
low libido
struggle with my weight
low enery
low mood
depressed out of my head
insomnia
anxiety
low libido
brain fog
and to some degree depersonalization but thats probably just sleep deprivation.

I did a little reading on adrenal problems and I do suffer from anxiety, had crippling panic attacks since 21 but I can control them now and I also have a skin condition, psoriasis. Also

Addison's disease symptoms usually develop slowly, often over several months, and may include:

Muscle weakness and fatigue
Weight loss and decreased appetite
Darkening of your skin (hyperpigmentation)
Low blood pressure, even fainting
Salt craving
Low blood sugar (hypoglycemia)
Nausea, diarrhea or vomiting
Muscle or joint pains
Irritability
Depression

Body hair loss or sexual dysfunction in women
Acute adrenal failure (addisonian crisis)


I have the ones bolded but they are hopefully due to low T and the fact I exercise regularly.

If I can just get my TRT prescription I would be so happy man, even do I know it can take months to see results, it would be a weight off my shoulders.

162nmol is about 4600ng/dl. I don't think you need TRT. The average range only goes up to about 40nmol (1150 ng/dl). You usually have to run 500mg of test weekly to get levels that high.

Sorry Megatron/guy's I wrote it wrong, I meant to write down 5.6nmol/l which is 162ng/dl as posted in the op.

Thats nothing new to me do constantly making mistakes and errors with my messed up head.

I've begun to notice that many guys on here with T problems, have anxiety or panic attacks. For the first time in my life, I had few panic attacks since I started TRT. If you don't mind answering this, could you tell me what helped you get over the panic attacks. Was it meds? a little help with some counselling? Some self education with books, etc? Just wondering what is most effective.
